DESCRIPTION
This IG defines focuses on the exchange of information related to social determinates of health (such as Food Insecurity, Housing Instability, and Transportation). The IG defines FHIR Profiles for Observation, Condition, Goals, Task, ServiceRequest, Procedure, Consent and Group resources to appropriately constrain these resources for use in documenting SDOH related care activities including: assessments, diagnosis, goal setting, intervention planning, intervention performing and communication with the patient.
