Great work from the ConnectedLife Health team to get this partnership over the line with Google, Fullerton Health & the Singapore Health Promotion Board! Excited to see the progress of this program into 2025, which aims to monitor Singaporeans lifestyle habits, through wearable devices, and provide real-time feedback and insights to help them prevent and/ or manage chronic health conditions.
Imagine a future where we can detect and manage chronic diseases before they even become a problem. That's the vision behind Singapore's Healthier SG initiative, and Google is proud to play a part. We're bringing the best of Google Health and Google AI to support three key initiatives in support of this vision: 1️⃣Together with Health Promotion Board, ConnectedLife Health and Fullerton Health, we’re piloting HealthTrack SG to explore how lifestyle changes can help manage chronic diseases. As you can see in the image below, we’re combining clinical data with Fitbit activity metrics, so patients can get an indication of their health wellness. Doctors will also get access to this information, which enables further personalized care and more effective consultations. 2️⃣Our support for the DigiCoach program, developed by Health2Sync 智抗糖 and Abbott, underscores our dedication to helping Singaporeans manage their glucose levels through informed lifestyle choices. 3️⃣ Together with ConnectedLife, we are supporting a research initiative, led by Alexandra Hospital, Singapore and National University Polyclinics to assess the effectiveness of wearables and digital coaching in encouraging healthier habits and managing chronic conditions.
