The Cardiovascular and Interventional Radiological Society of Europe (CIRSE) is a non-profit making, educational and scientific association aiming to improve patient care through the support of teaching, science, research and clinical practice in the field of cardiovascular and interventional radiology. CIRSE’s mission is to provide continuing education and training for its members, as well as other physicians and scientists with an active personal involvement and interest in interventional radiology and to promote the exchange of ideas and information for the further definition of the role, direction and goals of cardiovascular and interventional radiology as a subspecialty of radiology. We perform a wide variety of emergency interventions, some of them can be advanced - such as GI bleeding, ruptured AAA, or sometimes rescue TIPS. #CIRSEtakeover with Dr. Mateusz Krasun When things don’t turn out as they were planned Case by: Christian Smedberg and Niklas Nyman This is a 79-year-old male with hypertension and previous coronary artery intervention. He has a 55 mm AAA and a 26 mm common iliac aneurysm and was accepted for EVAR with iliac branched device to the right EIA/IIA. The iliac branched device was delivered as planned. Due to the angled infrarenal aorta it was decided to use an EVAR graft with suprarenal fixation. Figure 1 shows a type 1 endoleak which was treated with a proximal cuff that partially covers the right renal artery and also both left renal arteries (Figure 2). We used an Aptus introducer and stented all three renal arteries with Advanta 7x22 mm dx and two 6x22 mm sin. (Figures 3 and 4). A great kickoff to the liquid embolization course in Strasbourg today! Under the expert guidance of Prof. Christoph Binkert and Prof. Bernhard Gebauer, the day began with an in-depth exploration of the technical aspects of various liquid embolics. Participants then moved on to hands-on device training and in-vivo animal lab workshops. Tomorrow, they will have the opportunity to explore strategies for adapting when complications arise. #CIRSEtakeover with Dr. Mateusz Krasun Case by: Gustaf Lindberg and Niklas Nyman A 70-year-old male with a history of cirrhosis, resected hepatocellular carcinoma, and cardiovascular disease presents with abdominal pain and sepsis. CT shows widespread intramural gas in the proximal small intestine and gas in the portal veins, along with stenosis of the SMA and celiac trunk. During laparotomy, a large segment of necrotic small bowel was identified, with ischemic, non-necrotic segments at both ends, including the distal duodenum and proximal jejunum. The abdomen was left open, and an interventional radiology team placed a 6x26mm stent in the SMA to restore blood flow to the ischemic segments. Angiography showed improved flow. On second-look laparotomy, the affected duodenum and jejunum showed nearly normal color, though peristalsis was not fully restored. The necrotic bowel was resected, and short-term follow-up has been successful. #interventionalradiology #IRad #södersjukhuset

#CIRSEtakeover with Dr. Mateusz Krasun Cases by: Christian Smedberg, Viktor Mill, Niklas Nyman, Mateusz Krasun, Marteinn Smárason and Mareia Talvitie At Stockholm South General Hospital, interventional radiologists and vascular surgeons work together on both elective and acute cases. Although a general decline in the incidence of ruptured aortic aneurysms has been seen, we have treated three patients in the last month with emergency EVAR. All patients had challenging anatomy and needed occlusion with an aortic balloon to maintain adequate systemic circulation during the procedure. So far, the short-term outcome has been excellent. #interventionalradiology #IRad #södersjukhuset #vascularsurgeon #EVAR
