With over 500 members from 31 countries in 23 specialties; the ISSLS serves as a forum for the exchange of information of both an investigative and clinical nature relating to low-back pain and disability. ISSLS provides spine professionals with training and development opportunities, informational and networking events, and industry research and development opportunities.

#Neuropathic pain in the lumbar spine is associated with more severe neural compression and intervertebral disc degeneration. 🔥 The latest publication in European Spine Journal by our esteemed #ISSLS member Ashish Diwan + team Stone Sima and Sam Lapkin focuses on investigating the relationship between pain classification (nociceptive, neuropathic, or ambiguous) and lumbar intervertebral degenerative parameters obtained from imaging. Specifically, the study examines whether neuropathic pain in chronic low back pain (CLBP) patients is solely due to neuronal compression by correlating PainDETECT results with MRI and X-ray findings. The results suggest that neuropathic pain in CLBP is not only linked to neuronal compression but may also be associated with disk inflammation, revealing some crucial findings that could impact the future of #LumbarSpine research. This research underscores the significance of understanding the link between #PainClassification and pathological findings. In a field where low #BackPain and its underlying causes remain a major concern, these advancements are potentially critical for developing tailored treatment strategies and improving patient outcomes. Short Fixation with Penetrating Endplate Screws Enhances Stability and Function in #Osteoporotic Fractures Have you seen the latest publication in Spine Surgery and Related Research (#SSRR) by our esteemed #ISSLS member Kengo Fujii? This study introduces a novel technique for percutaneous vertebral augmentation using upward and downward penetrating endplate screws (#PES) and reports its preliminary outcomes. This is the first study to describe short fixation combining percutaneous vertebral augmentation with upward and downward PES. Given the challenges posed by poor bone quality in patients with unstable osteoporotic vertebral fractures, achieving a high degree of stability while improving functional outcomes is crucial. Key findings: 1️⃣ The proposed technique is straightforward and quick to perform, requiring only a short fixation range, and can be applied percutaneously without specialized imaging or navigation equipment. 2️⃣ The penetration of the disc space by PES may result in additional tissue damage and carries a risk of infection. 3️⃣ While the preliminary results are interesting, further studies with larger cohorts and longer follow-up periods are essential to validate these findings. Is there a way to speed up and/or promote “resorption” of a herniated disc that can eliminate pain quicker, avoid surgery and “not” administer anti-inflammatory medication or steroids as treatment? New study demonstrate that increasing #fatty #infiltration of the #paraspinal #muscle was directly correlated with a higher risk of undergoing surgery for lumbar #spondylolisthesis. The latest publication in the European Spine Journal by our esteemed #ISLSS member Murat Şakir Ekşi, is a groundbreaking study aimed to identify whether paraspinal muscle morphology could play a role in surgical decision-making in patients with lumbar spondylolisthesis. Given the emerging significance of paraspinal muscle pathology in intervertebral disc degeneration and chronic low back pain, these findings highlight muscle imbalance as a potential predictor of surgical intervention. Key findings: 1️⃣ Conservative and surgical groups were similar in terms of severe disc degeneration and Modic changes at any lumbar level. 2️⃣ Regression analysis and receiving operating curve analysis revealed an OR of 1.088 and a cut-off value of 17 points for fatty infiltration in the erector spinae to predict which patient could undergo surgery for lumbar spondylolisthesis. 3️⃣ Each 1-point increment in fatty infiltration in the erector spinae at any lumbar level increased the likelihood of surgery by 8%. Have you seen the latest publication in the Journal of Clinical Medicine (JCM) MDPI by our esteemed #ISSLS members Akinobu Suzuki, Shinji Takahashi, and Hiroaki Nakamura? Their groundbreaking study aimed to evaluate the functional outcomes and investigate the #RiskFactors for poor outcomes after palliative surgery for #spinal #metastases. This is the first study to demonstrate that hemoglobin levels are associated with functional recovery after surgery for spinal metastases. Considering the fundamental need to improve postoperative outcomes in already compromised patients undergoing surgery for spinal metastases, it is essential to recognize specific factors that could affect recovery trajectories following surgery. Key findings: 1️⃣ A lower Tokuhashi score and hemoglobin level were significantly associated with poor outcomes 2️⃣ The poor outcome group showed a significantly lower general condition and more extraspinal and spinal bone metastases 3️⃣ A lower general condition, a higher number of extraspinal bone metastases, a higher number of metastases in the vertebral body, and metastases to the major internal organs were independent factors associated with poor outcomes.
