Alliance CEO + Chair of Ontario’s Primary Care Collaborative (PCC) Sarah Hobbs alongside our PCC partners (Indigenous Primary Health Care Council, Ontario College of Family Physicians, Association of Family Health Teams of Ontario, Nurse Practitioner Led Clinics Association, and Ontario Medical Association's Section on General and Family Practice) met this week w/ Dr. Jane Philpott and the new Ontario Primary Care Action Team to discuss ways to collaboratively achieve a vision of 100% attachment to primary health care. We are excited to focus on key enablers of the vision with Dr. Philpott and her team, such as universal access to team-based care, centering equity, and the importance of wage parity for recruiting and retaining community primary health care staff and providers. The Alliance for Healthier Communities is the voice of a vibrant network of community-governed primary health care organizations in Ontario. Member organizations serve diverse communities across the province, and are rooted in the communities they serve. We share commitment to advancing health equity through the delivery of comprehensive primary health care. We are an organization and a movement built on the belief that health is “a complete state of physical, mental and social wellbeing and not merely the absence of disease and infirmity.” Through comprehensive primary health care, we aim to eliminate barriers that leave 3.5 million people in Ontario at risk of poor health. Together, we work to address the root causes of illness by changing social, economic and environmental policies to correct inequities harming people’s health.

Despite a “universal healthcare system”, too many people are left behind. Our latest work led by Parkdale Queen West Community Health Centre shows how mobile health clinics (MHCs) can fill critical gaps—bringing primary care, harm reduction, and mental health supports directly to people who need them most. The data makes it clear: Without bringing care to where people are at, nearly 40% of patients would have turned to the ER, and 1 in 5 wouldn’t have sought care at all. This isn’t just about access—it’s about rethinking healthcare delivery, meeting people where they are, and breaking down the barriers that keep underserved communities from getting the care they deserve. We know what works. Now, we need the policies and investment to scale it. Grateful to entire team with Laura Rosella and especially community partners for leading the way.

Workers in community health care across Ontario continue to face significant wage gaps. These wage gaps continue to make it harder to retain essential and dedicated community health staff who support health and wellbeing across Ontario, and difficult to fill vacancies in key roles across vital community health organizations and teams. To ensure we can reach the vision of each and every Ontarian being connected to primary health care, we need government leaders to commit to closing the gaps in wages that community health workers face across the province. Today is National Day of Remembrance and Action on Violence Against Women. Across Ontario and Canada, women and girls continue to experience violence and intimidation that have profound impacts on their lives and health, and of their children, families and communities. For racialized women and girls and others who face marginalization, misogynistic mindsets and actions perpetuate oppression and inequities. This must end NOW. We all have a responsibility to take action. On November 26 at Queen's Park, community health leaders from across Ontario shared a message of #equity and #access to increase primary care attachment w/ Ontario MPPs gathered for the Alliance's Community Primary Health Care Day. We showcased proven solutions to increase attachment to #PrimaryHealthCare and reduce overall health system costs. Rexdale Community Health Centre brought their mobile care bus right to the legislature from Doug Ford's home riding, to showcase what nimble, equity-driven approaches to increasing access to primary health care in Ontario can do. With the right support and funding, Ontario can overcome the crisis in #PrimaryCare and create a more sustainable, equitable health care system. #ONpoli #Healthcare #ONhealth
