Enabling patient success by connecting everyone along the patient journey along the continuum between home and hospital.
About us
We work to solve the fragmentation of healthcare. We are a team of highly experienced clinicians and technologists that have come together on a mission: SIMPLER and BETTER health care, through one digital platform that connects everyone to enable patient success. Why? Health care is already highly fragmented. New technologies that are being added fragment the health experience even further. This impacts the most patients with complex or chronic health issues, as gaps and overlaps are the main drivers of care dissatisfaction, poor health outcomes and skyrocketing health costs. How? Careteam's digital collaboration platform ensures that patients, families and health professionals manage health issues proactively and are on the same page. This improves the experience and outcomes of care while optimizing costs and use of resources. Careteam is a powerfully simple, cloud based and AI-enabled digital platform that enables care planning, communication and task management, and offers a set of tools that integrate the other technologies that people already use. Careteam enables clinicians to create care plans for patients with complex and chronic health issues that include instructions and recommended resources. The patient or their main caregiver can then share the care plans with who they see as their circle of care and Careteam enables the activation of the care plan into a to do action plan. Health organisations can monitor whether the plan is being actioned and proactively intervene to prevent deteriorations. Built by clinicians and patients and sold to health organisations focused on scaling innovation and accelerating their move to patient-centered care in the community, Careteam the only system that flexibly adapts to all situations and offers patients full control of who on their support network gets involved. You are seen, and you matter 🩵 Chronic complex conditions don't take a break for the holidays. The holiday season can amplify challenges, and many often feel even more down and alone. This is a shoutout to everyone who is just trying to make it through each day of this holiday season, dealing with their health, or those of loved ones, or mourning the loss of someone in their lives. Here are 12 ideas of how you can support someone who is in this situation: 1. Send them a quick message - text, chat, email - to say you are thinking of them, and simply remind them: You are seen, and you matter. We know it's though, and we are there for you. 2. Ask how you can best support them—what seems small to you might mean the world to them. 3. Share uplifting stories, music, or quotes to brighten their day. 4. Call and listen without offering solutions - sometimes, just being there makes all the difference. 5. If you're close, offer to sit with them or their loved ones so they can rest or step away for a while. 6. If you have a dog or a cat or a horse, offer to come by - pet therapy has shown to make a huge difference in supporting people in stressful times. 7. Volunteer to help with errands or take a task off their plate, like getting groceries, walking their dog or shovelling snow - small gestures can ease big burdens. 8. If they can't do it, take someone in their household who needs a bit of positivity out for a fun outing for a walk, coffee or the movies. 9. Invite them to join in holiday traditions, while making it easy for them to opt out or cancel if they need to. 10. Be a bridge to connection— offer to organize a virtual get-together or a phone call with loved ones, or an update to all of your friends and family. 11. Send or order for delivery a simple care package, a fun book or even a simple handwritten card to show you care. 12. If you can, donate to a cause or support service in their honor, showing solidarity with their journey. Remember: - Respect their need for space or quiet—check in gently without pressure. - Share resources and tips, only if they express interest. Sometimes these become just another to do on a long list. - Abstain from general sayings that have been proven to make people feel worse - such as "there is always a silver lining", "I am sure this is happening for a good reason", "just focus on the positive" or stories of your own or other people's challenges or misfortunes - Use validation and supporting statements instead, such as "this must be really tough", "you are incredible to be going through this", "I hope you are finding moments to breathe and do some self-care" - Above all, be patient and kind. For many people managing health challenges or caregiving responsibilities, the holidays can feel like just one more thing to navigate. Your understanding and support can be the light they need during this otherwise stressful season. 🩵

The most important things about the holidays season - and how we all define the true MAGIC of the season - has less to do with presents, trips and parties, and all to do with CONNECTION. We’ve all seen the incredible power of connection in action - strengthening relationships, driving better care, and improving lives - the bonds that link us, the shared understanding that unites us, and the relationships that empower us to create something greater together. We’re deeply grateful to everyone who we have worked with and for this year: patients, caregivers, healthcare providers, our customers and partners. Thank you for your continued trust in us and we so love all the great connections we have added this year! At Careteam, connection is at the core of everything we do: 🌟 Connection creates bonds, that empower patients and caregivers to feel supported and empowered in their care. 🌟 Connection fosters collaboration, bringing healthcare providers together to collaborate building on each other's strengths. 🌟 Connection builds pathways, uniting organizations into a more effective and efficient integrated care networks. 🌟 Connection inspires innovation, enabling health innovators to work together to create the kind of transformative solutions they couldn't create on their own. From all of us at Careteam, wishing you and your loved ones a holiday season filled with joy, peace, and meaningful old and new connections. Happy Holidays 🎉 🎉 🎉

Rob Attwell, our founder and COO, was at the Plug and Play Canada's Alberta Innovation Summit 2024 and presented to a selected audience of the best healthcare organisations and investors from Alberta and beyond, such as Alberta Blue Cross, Alberta Health Services, Bayer, Boehringer Ingelheim, Covenant Health Canada and Novartis. Rob presented how the Careteam platform is the integrated system that enables integrated care across the existing set of technologies that have been implemented, leveraging massive investments in interoperability, connected care efforts and Electronic Health Records (EHRs) such as EPIC in Alberta. Careteam takes this all to the next level, making it possible for: ✔️ Care plans based on evidence-based guidelines to be deployed by health teams for their colleagues, including those in different organisations, AND for their patients and family caregivers. ✔️ Collaboration and coordinated efforts, so everyone is on the same page AND can inform everyone who needs to know, reducing effort and cognitive overload. ✔️ Population health level insights, predictions AND actions enabling the health team to directly learn what works and then address any issues and gaps in care. The event was part of the Plug and Play Tech Center initiative as part of the Alberta Scaleup and Growth Accelerator Program, led by a consortium including Alberta Innovates, the Government of Alberta, Edmonton Unlimited, The Opportunity Calgary Investment Fund, and Prairies Economic Development Canada I Développement économique Canada pour les Prairies (PrairiesCan) - all aim to foster growth and innovation and drive entrepreneurship, applied research, and industry development, and we are very grateful to their support to the Plug and Play program and enabling us to participate. We are also very appreciative of the Plug and Play program as they connect innovative companies like Careteam with over 500 corporate partners, and investors to redefine HealthTech, as well as mentors, workshops, venture funding and collaboration opportunities. #digitalhealth #healthInnovation #PlugandPlay #integratedcare #patientsuccess

🌟 "Sometimes we have to let go of the life we have planned to make room for the life waiting for us." – Joseph Campbell. This profound quote resonates deeply with anyone navigating significant life transitions. For patients and caregivers facing life-altering illnesses, its meaning becomes even more poignant. When a diagnosis shatters the plans one has carefully crafted for their life and identity, it forces them onto a path they did not choose—a path filled with uncertainty, challenges, and often, profound transformation. A patient may find themselves navigating uncharted waters, redefining goals, and seeking hope in unexpected places. A caregiver, too, may step into roles that demand immense courage, resilience, and a reimagining of what their own life might become. Joseph Campbell, an American professor of literature, was renowned for his work in comparative mythology and religion. His most influential book, “The Hero with a Thousand Faces” (1949), examines the archetypal “hero’s journey” found in myths across cultures. Through his comparisons, Campbell identified universal themes in human experiences, demonstrating how these patterns connect us across time and culture. His ideas have significantly influenced modern storytelling and art, including iconic works like “Star Wars.” Campbell’s exploration of the “hero’s journey” reminds us that even in the face of unwelcome change, there is potential for transformation. Whether it involves discovering inner strength, building new communities, or finding purpose in advocacy, life after illness can stand as a testament to resilience and growth. This isn’t to romanticize the hardship—it is real, raw, and often overwhelming. Yet Campbell’s words highlight the potential for a new narrative to emerge within the pain. Both patients and caregivers, in their own way, embody the hero’s journey, finding ways to let go of the life they planned to make room for the life that awaits. The Careteam platform was designed to help health teams better guide patients and caregivers on that journey, creating a clear plan of action, and making it possible for others to support, help, advise, facilitate, and contribute as much as they can. And also learn from all the different journeys what new supports and services are needed. The hero’s journey is not easy, but can be made easier by getting others to help. And in many ways, the health teams working with us are also on a hero’s journey fighting the status quo, the complexity and the overwhelm and burnout to make a difference in their patient’s lives. What does this quote mean to you in your life journey? Let’s reflect and connect in the comments. 💬 #Resilience #Patient #Caregiving #LifeJourney #healthleadership #herosjourney

The 4th Hospital @ Home Leadership Summit organised by ICD Media Group starts today to Friday in San Antonio, TX with the theme “Accelerate the success of Hospital at Home Programs”. The "Hospital at Home" (HaH) model is revolutionizing healthcare by shifting acute care services from traditional hospital settings to patients' homes, in many ways using technology to make possible what used to be the norm - patients healing at home. The HaH approach has been proven to enhance patient comfort and experience, reduce demands on hospitals for beds and staff, and decrease exposures to communicable diseases, all while improving outcomes and costs. Careteam is a leading player in shaping the future of patient-centric, high-quality, and cost-effective at-home care and we are excited to see the increased attention to this area! It’s so important to learn from each other as a way to accelerate these efforts. Key challenges include managing the diverse needs of patients in non-traditional settings, early detection of and intervention for problems, ensuring seamless care coordination, and overcoming logistical barriers such as staffing, training, and resource allocation. Advancements in healthcare delivery, including remote monitoring technologies, telehealth solutions, and integrated care platforms, are paving the way for success. We are excited to hear from trailblazing leaders on the space, who are the top voices to follow, who have been pushing for the development of Hospital at Home for over two decades now such as Bruce Leff, MD at The Johns Hopkins University School of Medicine in the legacy of Dr. John Burton as well as (including so many MDs and RNs!): 💠 Mayo Clinic’s Michael Maniaci, MD 💠 Mass General Brigham’s David Levine, MD, MPH, MA and Stephen C. Dorner, MD, MPH, MSc. 💠 Kaiser Permanente’s Vivian Reyes, MD, Hemali Sudhalkar, MD, MPH, SFHM, Joeffrey Hatton, Ariel Herm, MPH and Tom Hackford, MD 💠 Cedars-Sinai’s Dr. Lindsay Williams, PhD, RN, NEA-BC, PHN, PMP 💠 Boston Medical Center (BMC)’s Tilar Martin-Asirifi, MS and Kate Beaudin, RN 💠 Atrium Health’s Daniel Davis, MD , Deena Denman, MSN, RN, CCRN, Jay Fraccola, BS, NRP, CP-C and Marvin Knight, MD 💠 UNC Health’s Michael C., MD and Rachelle Alpern, MBA, MHA 💠 Advocate Health’s Colleen Hole, BSN, MHA, FACHE 👉 Bonus: Does anyone remember STARPAHC (Space Technology Applied to Rural Papago Advanced Health Care) involving NASA and Lockheed, that demonstrated the feasibility in the 1970s of a consortium of public and private partners working together to provide medical care to remote populations via telecommunication? #hospitalsmovehome #hospitalathome #digitalhealth
