  • A 30-year-old with a history of chronic migraines and erythromelalgia presented with a red rash on their neck. The treating resident asked the Figure 1 community if they thought the diagnosis was a rash or levido reticularis. A number of dermatologists and rheumatologists on Figure 1 diagnosed the patient with erythema ab igne, a skin condition caused by heat exposure, and reminded the treating resident that the condition can be caused by repeated exposure to heating pads used to relax muscles and relieve headaches.   • Coming in at #2 for our top cases of 2024 is a patient with no notable medical history, post-COVID infection, experiencing shortness of breath, chest discomfort, dizziness, and hypotensive (SBP 85). According to the emergency medicine physician who shared the case, “ECG shows sinus arrhythmia with intermittent ‘runs’ of what appear like regularly occurring broad complexes.” Blood gas and cardiac enzymes are normal.   • As we head into 2025, we’re looking back at the top cases from last year. In case #3 Figure 1 members collaborate on a patient with severe, progressive foot pain following an E. Coli UTI and skin infection. Pain began in the third toe, spread to other toes and parts of the foot. Imaging shows joint effusion, edema, bone lesion, and nerve involvement.
