About us
Are you looking to use existing primary care data in a meaningful way? Access and use EMR data beyond your immediate practice to answer clinical questions and support your community with meaningful data? In a safe and secure environment? Managed by a trusted partner? The Health Data Coalition (HDC) is a physician-led data sharing initiative funded by the General Practice Services Committee (GPSC). The HDC supports a learning health system. Through its application HDC Discover, the HDC provides primary care physicians aggregate patient level data from individual physician practices, divisions, as well as province wide. This information can be viewed accross all the major EMRs used in primary care in BC. In so doing, physicians and divisions are able to reflect on their practice or community needs, and are also able to collaborate with their peers through private sharing. HDC Discover further supports self-reflective practice for the individual physician member in partnership quality improvement initiatives. HDC Discover is a flexible, accessible platform that provides HDC users access to a robust source of medical care data, while protecting provider and patient identity, and ensuring that physicians are always in control of their data. Together, we work with HDC users and partners to: • Unleash the untapped potential of EMR data from community physician offices • Connect and empower physicians with a better understanding of their practice, and support Divisions with aggregate trends in their community so to enhance patient care and contribute to a sustainable health care system • Remain actively engaged in building and improving HDC Discover through fair process, sustainable funding and transparent governance models.

Tips and Tricks – Access Screening Guidelines Through the References Tab. Screening measures help users identify patients with early symptoms of disease, where treatments may help in improving their outcomes. HDC Discover makes it simple for users to explore screening measures and access the guidelines that inform them. A key feature of the Single Measure View is the References tab, which provides links to approved guidelines to screening recommendations and evidence-based care practices. "If the frontline team wants to improve care for their frail clients, a good initial step would be to get a clearly defined Population of Focus (PoF). This would include using some queries to find likely candidates and then applying a standard coded problem in the project list…we keep our measure simple and unlock some features of our EMR, like the ability to get up-to-the minute data in reports or dashboards or apply batch changes to our PoF (e.g. send out an email questionnaire).
