About us

North York Family Health Team (NYFHT) proudly provides comprehensive and compassionate team-based primary care to its patients and community. We are constantly evolving to meet the needs of our community including how to serve our patients differently, how to support our team members through ambiguity, and how we can partner with others to make a difference collectively. We want to make a meaningful and purposeful impact in the lives of those we serve and the broader health system. We are among one of the largest team-based primary care organizations in the province, working alongside the Department of Family & Community Medicine at North York General Hospital and the University of Toronto, we provide to about 95,000 individuals, and as a member of the North York Toronto Health Partners (NYTHP Ontario Health Team), are committed to improving the health and wellness of our community.

A compassionate and caring interprofessional team of doctors, nurses, pharmacists, dietitians, social workers, chiropodist, and physician assistant who work collaboratively to partner with patients on their journey to better health and wellness.   • As we celebrate National Indigenous History Month it is a great opportunity to acknowledge Indigenous Peoples and to learn about their unique histories, cultures and ways of life.   At NYFHT, we celebrate diversity and are working towards creating a community where all people will be treated with respect, have their independence valued, and find greater access to integrated services. We are on a journey to better understand the needs of the populations we serve, and also take time this month to learn more about Indigenous Peoples.

  • Every June, 2SLGBTQI+ communities and allies come together to spotlight the resilience, celebrate the talent, and recognize the contributions of 2SLGBTQI+ people. Although special attention is put on Pride events during the month of June, they happen throughout the year in many communities. NYFHT is proud to be a supporter of equity and aims to ensure 2SLGBTQI+ rights. We take time this month to acknowledge and celebrate this diversity and reflect on how to build a world where every person is treated fairly and has the opportunity to achieve their greatest potential.

NYTHP and our partner, Flemingdon Health Centre, are #hiring a Senior Manager, System Transformation.   The successful candidate will be leading on our Ontario Health Team (OHT) Integrated Clinical Pathways (ICPs). They will be responsible for the framework approach, design, operationalization, evaluation and delivery of the OHT ICPs. Every February, people across Canada participate in Black History Month events and festivities that honour the legacy of Black people in Canada and their communities.   The 2024 theme for Black History Month is: “Black Excellence: A Heritage to Celebrate; a Future to Build”. This theme celebrates the rich past and present contributions and accomplishments of Black people in Canada, while aspiring to embrace new opportunities for the future.   In recognition and celebration of Black History Month (BHM), North York Family Health Team is encouraging its team members to participate in a series of programs to learn, celebrate, foster and discuss. Thanks to our partner North York General Hospital for the series of events.
