Currently, 20% of patients discharged from a hospital to a skilled nursing facility (#SNF) return to the hospital within 30 days of discharge, with each readmission averaging over $15,000 in costs. Our own Benjamin Kandel, Principal Applied Machine Learning Researcher, recently authored a study with JAMDA Journal about a new predictive model that identifies SNF patient hospitalization predictions over a 7-day period, using data from our EHR system. The results of the study showcase the utility of #machinelearning in the senior care industry and the benefits it can bring SNFs and the entire care continuum.
