“Modern AI is a HORIZONTAL ENABLING LAYER. It can be used to IMPROVE everything. It will be used IN everything. It is like electricity.” – Jeff Bezos. When clinicians and large device companies first heard about “smart” implants passively collecting objective data, analyzing it with algorithms, and transmitting findings to doctors and patients to interpret, automate, and personalize patient care, there were a lot of blank stares. Doctors said, “Why would I need to do that? My patients love me. I don’t have complications, and they WANT to see me.” Device companies were dismissive, “Too risky, too many steps, no business model.” Workhorse medical devices are rotary phones – born in the 50’s and 60’s, resistant to change, but reliable – if you are physically at home, the phone isn’t being used by anyone else, and you just need to make a call. There have been incremental (some would say “cosmetic”) design advancements in metal and plastic implants over the years, but we aren't in the flip phone era, much less the iPhone generation. However, in the last 8 months, there has been a seismic shift. Artificial intelligence, like electricity, has entered all aspects of life - even healthcare, the overachieving procrastinator of all industries. What will it look like in traditional MedTech? For the practice, AI dramatically improves workflow. Post-operative total knee arthroplasty (TKA) care is one of the most time-consuming and labor-intensive activities in medicine. Multiple surgeon visits, ten or more PT treatments, periodic radiographic studies, homecare – episodic assessments to identify poor performers and implement corrective rehabilitation. Even patients recovering normally (>80%) get multiple examinations, reassurance, and encouragement to adhere to rehabilitation protocols. As for the >50% of patients lost to follow up? 🤷🏻♂️ The whole exercise is completely unscalable in a high volume, high throughput, personnel-constrained healthcare system. AI-Driven Post TKA Care automates a manual process: Implantable Sensors continuously, passively collect Objective Data → Activity, ROM, and Gait → Data Transmitted to Cloud → AI Analyzes Recovery Trends (Recovery Curves) → Identifies Outliers (Sort & Filter) and Potential Complications (Diagnostics) → Generate Custom Recovery Plans (PT & Exercises) → Deliver the Plan to the Patient. The care team must oversee and verify the findings - but it is more efficient than generating findings.   To the patient, Medical AI is like interacting with ChatGPT (see the figure below). A ChatBot provides personalized physiotherapy plans, makes recommendations, and answers questions – based entirely on THEIR data. We commit to having this in 2025. Industry experts are predicting all medical devices will leverage AI within 5 years. Every venture capitalist saw it coming years ago. Big MedTech has had internal AI programs for over a decade. Revisionist history is a beautiful thing. Let the electricity flow.
