Several factors make stent implantation unattractive in this situation: chronic total occlusion, 1:1:1 bifurcation, calcifications, small diameter (< 3.0 mm) and very long lesion. After crossing with a Gaia 2 wire, wolverine cutting balloon was used in the LAD & Diagonal. Protégé NC paclitaxel coated balloon was applied to LAD and Diagonal achieving a great angiographic and OCT result. The presence of a media dissection indicates luminal gain in the near future. Protégé NC has an excellent profile and is a great choice to treat complex lesions. We will soon launch a new trial focussing on the efficacy of Protégé in complex coronary lesions. 📣 A Very successful Workshop on Complex Coronary Interventions in German Heart Centrum, Munich! The SIS Medical team is pleased to announce an other milestone of our Complex Coronary Interventions Workshop. We successfully conducted the last workshop for 2024 at the German Heart Center in Munich on November 27-28. Lead by Dr. Salvatore Cassese and Dr. Erion Xhepa, this workshop was dedicated to discuss the increasing difficulties in treatment of patients with challenging lesions. The Complex High-Risk and Indicated PCI (CHIP) program showcased different techniques and tools designed to improve long-term patient outcomes. - #ComplexPCI: Recognizing the complexity of planning and execution. - #ComplexAnatomy: Strategies for navigating tortuous and heavily #calcified lesions. - Treatment Algorithms: Approaches for managing non-crossable or #undilatable lesions. - Advanced Techniques: Implementation of different plaque modification modalities. How to deal with coronary calcified lesions, which can not be dilated with a regular NC balloon? Max Maffey and Rodrigo Bagur give a comprehensive overview about Dedicated Balloon Techniques for Coronary Calcium Modification Challenge: #CoronaryCalcification - represents a significant technical challenge in #PCI - is estimated in approximately one-third of patients undergoing PCI - can increase the risk of stent underexpansion and malapposition - can increase complications such as perforation and dissection - is associated with an increased risk of repeat revascularisation, stent thrombosis and death Challenge: #Undilatable calcified lesions, using a regular NC balloon Secco et al. retrospectively assessed the efficacy of OPN NC balloons in 91 patients with calcified lesions that could not be dilated with a regular NC balloon. Percent diameter stenosis was noted to be significantly lower after OPN ballooning compared with the NC balloon (41.1 ± 15.8 versus 20.2 ± 14.6; p<0.001). Prospective, randomised data are also available to support the use of super high-pressure NC balloons. The ISAR-CALC trial (Priv.-Doz. Dr. Tobias A. Rheude et al.) randomised 74 patients with severely calcified lesions with unsuccessful NC balloon pre-dilatation (defined as a <30% reduction in baseline diameter stenosis) to undergo lesion preparation with either a super high-pressure balloon or a scoring balloon. Compared with the scoring balloon, super high-pressure ballooning increased the minimum lumen diameter (2.83 ± 0.34 mm versus 2.65 ± 0.36 mm; p=0.03) and reduced the diameter stenosis (11.6 ± 4.8% versus 14.4 ± 5.6%; p=0.02). Strategy success, defined as target lesion residual stenosis <30%, Thrombolysis in Myocardial Infarction (TIMI) score 3 flow and no major adverse cardiac events achieved without the need for additional devices for lesion preparation, was also high in the super high-pressure balloon group at 91.9%. In one retrospective study (Secco et al.) of 326 patients with calcified coronary lesions undergoing PCI with OPN NC balloon, perforation was reported in 0.9%. Therefore, complications secondary to super high-pressure balloon inflation appear to be relatively uncommon. This safety is mainly dependent on the recommended avoidance of 1:1 reference vessel dilation but rather a 0.5 to 0.7 ratio, similar to that recommended for cutting balloons. #OPNNC #CCL #CalcifiedCoronaryLesions SIS Medical https://lnkd.in/eYvswACc
