DynaMesh® International Hernia Masterclass Summaries Episode 04 In his lecture “Mesh Development and Production”, Dr.-Ing. Andreas Müllen, R&D Director at DynaMesh® - FEG Textiltechnik, follows the excellent research by Prof. Bernd Klosterhalfen and gives a brief overview of the history of DynaMesh® - FEG Textiltechnik and its fundamental research activities that drive surgical mesh implant innovation with dozens of collaborative research projects with world-renowned medical (e.g. Uniklinik RWTH Aachen), scientific (e.g. DWI - Leibniz Institute for Interactive Materials) and engineering institutions (e.g. ITA Institut für Textiltechnik der RWTH Aachen University). The basic requirements for every surgical mesh implant intended for permanent bridging and reinforcement are: a long-term stable material, high biocompatibility, low foreign body reaction, and mesh implant properties such as strength, elasticity, and porosity. Essential for the success of any mesh-based surgery is adapting these parameters to the specific application, i.e. the Tailored Approach. As outlined above, the implementation took place in close interdisciplinary cooperation between users, researchers, and developers. Consequently, the DynaMesh® implants were developed using the Tailored Approach to meet clinical needs with suitable innovative textile solutions. DynaMesh® International Hernia Masterclass Summaries Episode 03 Another highlight of this year's DynaMesh® International Hernia Masterclass was the impressive presentation by Prof. Bernd Klosterhalfen on mesh materials. Prof. Klosterhalfen has been working with mesh implants for over 30 years as a clinical pathologist and has unique expertise based on the analysis of thousands of explants, which today constitute the world’s largest database of explanted mesh implants. The latest findings from studies on the subject of ageing resistance/degradation were presented. For example: ➡️ In an in-vitro study, in which the U.S. Food and Drug Administration, among others, was involved, the oxidative degradation of polypropylene (PP) mesh implants was clearly demonstrated by automated accelerated degradation testing over 4 weeks [1]. ➡️ The results of a comparative animal study on the ageing resistance of PVDF and PP were presented. Over a period of two years, progressive degradation of the PP mesh implants was demonstrated in vivo, while the compared PVDF product showed no signs of degradation after the same implantation period [2]. ➡️ In a recently published animal study by The University of Sheffield, the concentration of degradation-associated PP particles in the surrounding tissue around a PP mesh implant was quantified for the first time. A concentration of PP particles was already clearly measurable after 2 months, increasing more than tenfold in the period between 2 and 6 months. Further effects of degradation were an increase in the roughness and size of the mesh surface and an increase in the implant stiffness [3]. In summary, PVDF is long-term stable and long-term inert, whereas PP degrades over time due to oxidation, which has a clinical impact. Jens Plambeck had compiled short video clips overnight from the previous day's live surgeries, highlighting the most critical scenes from each operation from the surgeons’ perspectives: Miguel Ángel García Ureña Nihad Sardoschau Dieter Berger PERICLES CHRYSOHERIS, MD,FACS Grigoris Chatzimavroudis Hakan Gök, MD, FEBS AWS Dr. med. Konstantinos Zarras These clips were reviewed and discussed with more than 200 experts in the auditorium and over 400 online participants. A highly productive exchange of experiences took place between the experts on stage and the professionals from over 20 countries in the auditorium. Suggestions, experiences, literature, as well as tips and tricks for the respective surgical methods, were shared and discussed. Alternative surgical approaches were also debated. 📣 News on DynaMesh®-SIS direct 📝 The study from Mansyet al. showed that the DynaMesh®-SIS direct group had SHORTER OPERATIVE TIMES, SHORTER TIME TO RETURN TO NORMAL ACTIVITY and SIGNIFICANTLY FEWER CASES of PERSISTENT POSTOPERATIVE PAIN compared to the hybrid sling. Read more below: Stress urinary incontinence is a chronic and debilitating disease with a significant impact on women’s quality of life. A new randomized clinical study published by Mansy and colleagues compared the safety and performance of two types of transobturator slings that are inserted underneath the urethra for support: the synthetic DynaMesh®-SIS direct sling made of polyvinylidene fluoride and a hybrid sling made of polypropylene and autologous fascia (BioArc sling system, AMS). The authors reported excellent cure rates of more than 93 % in both groups at 12 months after surgery. Post-operative urgency and urge incontinence rates did not significantly differ between the groups. There was one case of erosion in the DynaMesh®-SIS direct group and no case of erosion in the hybrid group, a difference that was described as “clinically insignificant” by the authors. In addition, the study showed that the DynaMesh®-SIS direct group had shorter operative times, shorter time to return to normal activity and significantly fewer cases of persistent postoperative pain compared to the hybrid sling. DynaMesh® International Hernia Masterclass Summaries Episode 01 We start with the first part of the scientific programme on “Anatomy of the Abdominal Wall - Challenges in IPOM and Extraperitoneal Techniques”, a great presentation by Dr Alexis Theodorou. In his lecture, he discussed the anatomy of the abdominal wall and took the audience on a journey with Medicalholodeck: A spatial application for surgical training using digital twins. With the help of this tool, Dr Alexis Theodorou impressively illustrated what Prof. Miguel Ángel García Ureña recently published*: The extension of the fatty triangle to the fatty trident, which extends to the pubis and, via the Borgos space, far to the lateral side. The concept of the preperitoneal fatty trident may be of practical assistance to perform various hernia procedures, as it facilitates the preparation of the preperitoneal space.
