An interesting case of an ischemic ventricular tachycardia in a patient with previous missed STEMI and a severe inferobasal left ventricular aneurysm. Preprocedural late iodine CT scan segmented with the inHEART software revealed wall thinning in the aneurysm and similar subendocardial/transmural scar. Late Annotation Mapping, Bipolar Voltage map and VT Activation map correlated nicely with channels found in the CT scan. Ablation was performed with focal pulsed field electroporation. Patient was non-inducible post ablation. Focal PFA of a PVC originated from the right ventricular outflow tract. Multipolar activation map with the Optrell mapping catheter revealed some nice fragmented prepotentials above the pulmonary valve. Perfect pace correlation was also found at the same site. A beautiful incisional right atrial atypical flutter. The patient had previous surgery for atrial septal defect and the flutter circuit was found around the incisional scar. Critical isthmus was found between the scar and the crista terminalis. The flutter terminated on the first pulsed field ablation. Nice case of a septal accessory pathway with ERP 250 ms during isoprenaline. The pathway was mapped during distal CS pace. Earliest activation was found at the anterior CS ostium, with very nice signals on the Optrell mapping catheter. The activation map revealed initial conduction over the pathway followed by conduction over the His/purkinje. Pathway was ablated with radio frequency (25 Watts). Bi -Atrial atypical flutter mapped with Carto V8. The flutter was mechanically induced after pulmonary vein/posterior wall isolation and an anterior mitral line with focal pulsed field ablation. The flutter terminated on the first ablation over the Bachmanns bundle insertion in the left atrium. No further arrhythmias was induced. Testing some new tools in the CARTO v8 toolbox. 🛠️😎 In this PVI case we tried the Cartosound fam module with automatic delineation of the left atrial structures. Fast mapping and very nice definition of the LSPV/LAA ridge. Minimal to none editing needed. We compared the model with an octaray fam and the CT scan, which lined up nicely.
