To support and create an environment for good health and wellbeing, there are many pieces that contribute to that puzzle. We need meaningful public health measures, we need holistic access to primary care, we need clean communities, and this list goes on. Within the research community, there are many committed researchers tackling these various challenges from different angles. In ESCAPE, we are investigating opportunities to make care more streamlined for patients, this means getting the right care in the right order, this means supporting in setting micro-health goals and this means utilisation of care managers. In ESCAPE, these care managers, typically nurses, interact closely with the patients, to improve their quality of life and wellness. Our randomised controlled trial (RCT), led by Professor Christoph Herrmann-Lingen at Universitätsmedizin Göttingen, is making incredible progress, we've hit 90% of our recruitment target! This trial compares ESCAPE’s innovative blended collaborative-care approach with current care for multimorbid patients with chronic heart failure and psychological distress. We’ve successfully screened over 3,500 patients, with about 90% of our target randomised into the trial. With a target of 300 patients across Germany, Denmark, Italy, Hungary, and Lithuania, this marks a pivotal moment in our journey. 📢 New Publication Alert! We are thrilled to announce the release of our latest study: "Care manager role for older multimorbid heart failure patients’ needs in relation to psychological distress and quality of life: a cross-sectional study". This research explores the crucial role of care managers in addressing the unique needs and preferences of elderly heart failure (HF) patients with multimorbidity. By utilising the Blended Collaborative Care (BCC) approach from the ESCAPE project, we identified key areas where care managers can significantly improve patient outcomes, including personalised treatment, education, and symptom monitoring. Key findings: 🔹 Psychological distress and lower quality of life were common challenges for elderly multimorbid HF patients. 🔹 A personalized, patient-centered approach is essential for addressing these needs and improving overall care. This study underscores the importance of care managers in providing tailored care to enhance the quality of life for elderly HF patients, particularly those experiencing psychosomatic distress.
