Developed as part of the CAREMATRIX project, INCA is a significant step forward in the digital transformation of healthcare in Norway. ✨What is INCA? INCA offers a comprehensive solution for the early identification and management of Patients with Multimorbidity (PMM) through a multidimensional assessment using personalized questionnaires. This tool integrates data from various sources—social, health, and physical activity information—to create a holistic view of each patient's needs. The platform also enables the creation and management of Shared Care Plans (SCP), facilitating real-time coordination among different professionals, with role-based access to information. It includes an alert system for early detection of decompensation and an AI-based Clinical Decision Support System to optimize patient care. Finally, INCA includes a dedicated app for patients, caregivers, and family members. Ready to excel in Phase 3! 🚀   Last week we had the pleasure to start the testing and piloting phase of the CareMatrix project, doing a successful Kick-off meeting at Innovation Skåne office in Brussels.     We are very grateful to count on International Foundation for Integrated Care (IFIC) to collaborate in dissemination actions and to review the business strategy altogether with Norway Health Tech and BIOEF.   We continue to move forward with enthusiasm into the future!   #Innovation #HealthTech #Collaboration #Phase3

🚀 INCA Healthcare, the technological solution presented by Doole Health and with the help of partners Germans Trias i Pujol Research Institute (IGTP), Universitat Oberta de Catalunya, Norwegian Centre for E-health Research, Fundació Lluita contra les Infeccions, has been selected to move on to Phase 3 of the Carematrix! 🎉    During this Phase 3, we are very proud to be able to validate our solution in three environments: Norway, Sweden and the Basque Country. We would like to announce that, on 19 June, we demonstrated our INCA product to future Buyers.    We were left with a very good taste in our mouths and we are looking forward to move on to Phase 3 of CareMatrix to be able to evaluate our solution in pilots with end users, and thus get even closer to making it a reality for the monitoring of multimorbid patients.   #CareMatrix #HealthInnovation #MedicalCare
