💡 All hope is not lost – tackling the global kidney disease burden with new therapeutics 🌍 Kidney diseases now affect over 850 million people worldwide, and the numbers are steadily increasing, driven in part by the rising prevalence of diabetes. This growing burden places significant strain on healthcare systems globally (1). Despite the growing burden, all hope is not lost. Recent therapeutic advancements, particularly the dual actions of #SGLT2 inhibitors and #GLP-1 receptor agonists, have shown remarkable improvements in cardiovascular and kidney outcomes. 💡 Tetracycline antibiotics may be used for up to three weeks also when breast-feeding   Use of systemic tetracycline antibiotics during breastfeeding has long been of concern due to risk of dental staining of the breastfed infant associated with long-term use, such as for treatment of maternal acne. These drugs are excreted into breast milk in relatively low amounts, and, e.g. for doxycycline, it has been shown that simultaneous ingestion of milk reduces the absorption by up to 50% (1). It appears from literature searches that substantial new information has not been published lately, but the apparent lack of reported adverse effects has led to gradual prolongation of the recommended maximal length of use from one week (2) to the current practice of 3 weeks, as also used by the Helsinki University Hospital Teratology Information Service. 💡 At least one actionable pharmacogenetic mutation is present in 97-99% of population (1,2). More than half had three or more mutations (2). Drugs that are affected by these variants had been prescribed to 31-37% of these patients (1,2), two prescriptions to 25% of patients, and three prescriptions to 12% of patients during follow-up (2). Many commonly used drugs are pharmacogenetically sensitive. Ten most frequently prescribed drugs were simvastatin, tramadol, ondansetron, codeine, citalopram, clopidogrel, allopurinol, warfarin, amitriptyline and paroxetine (2). 💡 Herbal products containing ashwagandha and turmeric/curcumin linked to liver injury   Herb-induced liver injury (HILI) is a notable cause of acute liver dysfunction, with herbal products and supplements responsible for 16-20% of drug-induced liver injury (DILI) cases (1-4). Recent studies have linked ashwagandha and curcumin (from turmeric) to liver injury. Twenty-two cases of severe liver damage have been associated with ashwagandha, and at least 48 cases with curcumin, especially when combined with piperine to enhance absorption. Symptoms include jaundice, nausea, and elevated bilirubin levels, and while liver function generally returns to normal after discontinuing use, three deaths have been reported.
