Sociabble was asked by 101 Fund, the first international endowment fund dedicated entirely to the progress of intensive care, to help them create their own digital communication platform. In order to do this, the SECOND LIFE digital network is exclusively dedicated to supporting intensive care survivors, as well as their loved ones and caregivers, to "live better" during and after intensive care. The platform also connects all caregivers in the field and seeks to bring enhanced meaning and purpose to their daily lives. 👉 One O One's objective? To save 1 million lives over the next 4 years, by fulfilling 3 major missions: better care, better research, and better support. Journée Mondiale de l'#Amylose L'amylose est une maladie rare et complexe, caractérisée par l’accumulation anormale de protéines et qui forment des accumulations dans divers tissus et organes, provoquant des dysfonctionnements, des défaillances organiques, et dans certains cas le décès. Malgré les défis persistants, la recherche scientifique a fait des avancées significatives, soulignant l'importance d'une prise en charge rapide pour améliorer la qualité de vie et la survie des patients. La Journée Mondiale de l’Amylose vise à unir tous les acteurs concernés pour renforcer la recherche et le soutien aux patients. Fière membre de l’équipe qui a remporté l’édition 2024 de #ideathon de MEDFAR avec une idée toute simple visant à désengorger les urgences. Une idée toute simple mais qui aurait le potentiel d’influer sur les bounce-back à l’urgence. Beaucoup l’ignorent mais l’absence de corridors d’information directs entre les patients ayant congé de l’hospitalisation et les GMF/ Première ligne conduit à autant d’occasions manquées d’agir en amont d’une nouvelle hospitalisation. Près d’un patient sur 10 qui aura son congé de l’hôpital sera hospitalisé dans les 30 jours suivants son congé. Parmi ceux qui sont revus à l’urgence dans les 7 jours suivant leur congé, 1/3 sera hospitalisé à nouveau. Désengorger l’urgence, c’est se donner les moyens d’intervenir pour prévenir la décompensation de ceux qui sont le plus à risque. Et la clé: le partage de l’information pour activer les interventions en amont de l’urgence. Les GMF et les mdf sont trop svt les derniers à savoir que leurs patients ont été hospit et il n’existe actuellement aucun corridor de priorisation au GAMF pour les orphelins qui retournent en communauté. SAD et GMF ignorent trop souvent qui sont les pts qu’ils ont en commun. Nous avons imaginé un outil tout simple - HComm - qui permet d’activer tous les intervenants et combler le fossé entre l’hôpital et la communauté (et nous avons gagné ;) 🙌 Souvent il ne s’agit pas de réinventer la roue… faut juste s’assurer de gonfler les pneus
