SRS for Multiple BRAIN METASTASES (BMs): <10 BMs versus >10BMs

STEREOTACTIC RADIOSURGERY and STEREOTACTIC RADIOTHERAPY for BRAINSTEM METASTASES: An international multicenter analysis. Alexander Muacevic et al. Int J Cancer. Sep. 2024. https://lnkd.in/dKg-KCKv International multicenter study investigates the efficacy and safety of SRS and FSRT. 136 patients with 144 BSM median Radiographic and Clinical Follow-Up periods were 6.8 and 9.4 months, respectively. - median Prescription Dose for SRS were 18 Gy and Isodose line and 65%. - median Prescription Dose for FSRT was 21 Gy with a median Isodose line of 70%. 12-, 24-, and 36-month local control (LC) rates were 82.9%, 71.4%, and 61.2%, respectively. 12-, 24-, and 36-month Overall Survival (OS) rates were 61.1%, 34.7%, and 19.3%. - minimum Biologically Effective Dose (BED) was significantly associated with LC, favoring Higher Doses for improved control ( p < .01). - Good performance status (Karnofsky performance status, ≥90%; (p < .01) and prior whole brain radiotherapy (p < .01) associated with improved OS Treatment-related adverse events: five (3.4%) Radiation Necrosis. Authors concluded that SRS and FSRT for BSM exhibit efficacy and safety, making them suitable treatment options for affected patients. #SRS #radiosurgery #brainstem #metastases #braintumor

REPEAT STEREOTACTIC RADIOSURGERY in VESTIBULAR SCHWANNOMA patients: A systematic review and meta-analysis Bardia Hajikarimloo et al. J Clin Neurosci. 2024 Dec. https://lnkd.in/d6Vd5Jh3 11 studies encompassing 260 VS patients with repeat SRS median TIME INTERVAL between initial and repeat SRS from 43 to 62 months * TUMOR CONTROL was 91 % (86 %-94 %). * RADIOLOGICAL RESPONSE: - Regression rate was 59% (52 %- 65 %) - Progression rate was 9% (6 % - 14 %) * CLINICAL OUTCOMES: - Serviceable HEARING preservation (SHP) rate was 36% (22% - 53%) - worsened FIFTH cranial nerve (CN) were 12% (7% - 19%) - worsened SEVENTH CN rates were 8% (5% - 12%) * Adverse Radiation Effect (ARE) rate was 6% (3% - 11%) Conclusion: Repeat SRS following the failure of the initial SRS in VS is associated with favorable outcomes, including tumor control, SHP, and CN worsening concurrent with low ARE rates. #acousticneuroma #Stereotactic #Radiosurgery #SRS #Vestibular #Schwannoma.

Systematic Review and Meta-analysis on SRS for the management of Growth Hormone (GH)-secreting pituitary adenomas. 346 abstract reviews, 27 retrospective studies. RESULTS: - Remission rate between 40% and 60% - Overall endocrine remission estimate after SRS were 46% (95% CI: 39-53%). - 5-year probability of remission estimate after SRS were 48% (95% CI: 38-57%). - New hypopituitarism estimate after SRS was 23% (95% CI: 17-29%). - Radiation induced Optic Neuropathy after SRS ranged between 0% and 6% This meta-analysis confirms and quantifies safety and effectiveness of SRS to achieve endocrine remission after surgical management in acromegaly. #SRS #radiosurgery #pituitaryadenoma #acromegaly #GH #growthhormone

HYPOFRACTIONATED SRS (Stereotactic Radiosurgery) in the Management of BRAIN METASTASES Eric J Lehrer et al. Neurosurgery. 2024. https://lnkd.in/dfkyiyvY Single-fraction SRS is associated with high local control rates ranging from 70% to 100%, dependent on lesion and postoperative cavity size. When treating LARGER Targets and/or targets located in ELOQUENT Locations, the risk of normal tissue toxicity and adverse radiation effects within healthy brain tissue becomes significantly higher. Hypofractionated SRS allows for the delivery of ablative doses of radiation while also minimizing the risk of toxicity. There are NO REPORTED RANDOMIZED data to date, but there are trials underway evaluating this paradigm. In this article, authors review the role of hypofractionated SRS in the management of brain metastases and emerging data that will serve to validate this treatment approach. #SRS #radiosurgery #brainmetastases
