Almost all #FHIR artifacts can be assigned a #maturity level, from 0 to 6. Understanding the FHIR Maturity Model (FMM) is essential for anyone involved in healthcare data exchange. FMM provides a clear framework for assessing the readiness and stability of FHIR artifacts like resources, profiles, and implementation guides. Here's a concise guide to what each FMM level represents: FMM Level 0: The artifact has been published on the current build but is still a draft. It must have a standards status of Draft. FMM Level 1: Building on Level 0, the artifact produces no warnings during the build process. The responsible work group considers it substantially complete and ready for implementation. For resources, profiles, and implementation guides, the FHIR Management Group has approved the underlying proposals. FMM Level 2: The artifact has been tested and supports interoperability among at least three independently developed systems. These systems leverage most of the scope (at least 80% of core data elements) using semi-realistic data and scenarios, such as those tested at a connectathon. The results have been reported to and accepted by the FHIR Management Group. FMM Level 3: Verified by the work group as meeting the Conformance Resource Quality Guidelines, the artifact has undergone formal balloting. It has received at least 10 distinct implementer comments from at least three organizations, resulting in at least one substantive change. FMM Level 4: The artifact has been tested across its scope, published in a formal publication (e.g., as a Standard for Trial Use), and implemented in multiple prototype projects. The responsible work group agrees that any non-backward compatible changes will require implementer consultation due to the artifact's stability. FMM Level 5: Published in two formal release cycles as a Standard for Trial Use, the artifact has been implemented in at least five independent production systems across more than one country. FMM Level 6 (Normative): The pinnacle of maturity, the artifact is ready to be locked down according to inter-version change rules. It has passed the HL7 normative ballot, and both the responsible work group and the FHIR Management Group agree it achieves Normative standard status. Why FMM Levels Matter The FHIR Maturity Model is more than just a scale—it's a tool that guides the healthcare industry toward robust and interoperable solutions. Outburn Ltd. is pleased to announce a new collaboration with the MAR network. MAR provides medical imaging and diagnostic services, with 48 branches spread across the country. With over 40 years of experience, MAR performs over 700,000 scans annually for all four HMOs, including X-rays, CT scans, mammography, ultrasound, EMG, and more. Outburn will deliver a solution for the implementation and verification of service authorizations via Form 17. The solution enables the management of this process with all four healthcare funds and includes the development of an integration component with a management interface that handles the entire process end-to-end: from receiving requests from MAR’s EMR, calling the health fund’s service, receiving a response, and sending it back after conversion. The FUME solution will handle conversions between MAR’s EMR structure and the FHIR CoverageEligibilityRequest resource, as well as write the data back to the EMR. This solution lays the foundation for future FHIR-based integration applications for MAR. We would like to express our appreciation to MAR for their initiative in creating an interoperability process with the healthcare funds. This collaboration is another step towards building interoperability in Israel, which will reduce waiting times at the institute and thus improve the patient experience. Additionally, adopting the solution will lead to operational improvements by reducing clerical time and, consequently, operational costs, both for MAR and the HMOs. Yesterday, we had the privilege of participating in an enlightening community seminar focused on SMART on FHIR, a cornerstone of modern healthcare IT infrastructure. SMART is a framework that allows developers to build applications that can access and interact with healthcare data stored in FHIR servers. SMART on FHIR is a game-changing standard that enables secure, seamless integration of healthcare applications with electronic health records, promoting unprecedented levels of interoperability and innovation in the healthcare technology landscape. The seminar brought together a diverse audience, including representatives from healthcare organizations, established vendors like InterSystems and Amazon AWS, and innovative startups. This mix of participants created a rich environment for knowledge exchange and networking. We were fortunate to hear insightful presentations from industry experts, including Tani Frankel and Keren Skubach from InterSystems, as well as representatives from Amazon Web Services (AWS). Throughout the day, we explored the various components of the SMART on FHIR standard, gaining a deeper understanding of its benefits and the challenges involved in its implementation. The speakers provided valuable insights into how SMART on FHIR can transform healthcare delivery and data management. The highlight of the event was the hands-on session at the end of the day, where we successfully built and ran our own SMART application. This practical experience reinforced the concepts we'd learned and demonstrated the accessible and powerful nature of SMART on FHIR technology. In the ever-evolving world of #FHIR, one of the biggest challenges is maintaining backward compatibility while integrating the latest features from newer versions. Cross-version extensions offer a simple but effective solution to this problem, allowing us to use elements from FHIR R5 in R4 implementations by assigning unique extension URLs. This can prevent the need for creating custom extensions for elements that HL7 has already come up with. In fact, the specs demand this solution be used when available! Each cross-version extension has its own URL, according to the following template: http://hl7. org/fhir/[version]/StructureDefinition/extension-[Path] where [version] is the version number of FHIR (e.g., 4.3, 5.0 etc.) and [Path] is the element path. This allows implementers to incorporate new elements without having to shift-up the FHIR version of the entire project. Just recently, we required such a cross-version extension on one of our projects, where we required the R5 version of DiagnosticReport.note element for an R4 implementation, where this element doesn’t exist, nor is there a canonical extension for it. To do this, the URL required was: http://hl7. org/fhir/5.0/StructureDefinition/extension-DiagnosticReport.note Note that the [version] is 5.0.
