It is not uncommon for my practice to see patients with Large Macular Holes. There are 2 reasons for this! 1. Often these cases are not diagnosed by the primary care giver (an optometrist or General ophthalmologist (read "cataract surgeon")! As Spectral Machines get cheaper and more widely available, in my part of the world, i am hopeful this issue of "missed Diagnosis" will become uncommon in the future! The second reason is that, many of these primary care givers have the misconception that "Large Macular holes" or "Old Macular Holes" don't gain from treatment. Not much is being done to change this situation. In our local retina meetings discussions are more often about "medical Retina" conditions and "injection protocols". I feel we need to do more to create awareness about Recent Advances in "Surgical Retina". Advances like; Updates in Techniques (ILM Flap for Large Holes) and Machines (The Multicolor light system of OS4 which we have in our centre, that make ILM peeling feel like a "Walk in the Park") need to be shared!
