AbstractAbstract
[en] Objective: To determine the coagulation abnormalities and relationship between abnormal clotting tests and the risk of gastrointestinal bleeding (GI) among chronic liver disease (CLD) patients admitted at a tertiary care hospital in Pakistan. Methods: Adult CLD patients admitted at Liaquat University Hospital Jamshoro, during Nov 2004 - Oct 2005, were included in the study. The patients blood were tested for coagulation abnormalities including prothrombin time (PT), activated partial thromboplastin time (aPTT), platelet count and plasma fibrinogen. Association was seen between the abnormal clotting tests and the gastrointestinal bleeding by calculating relative risk (RR) with 95% confidence interval. Results: PT was prolonged in 88% and aPTT was raised in 71% cases of CLD. Both PT and aPTT were prolonged in 67% CLD cases. Approximately 37% CLD cases had decreased platelet count and 15% cases had decreased serum fibrinogen level. Relative risk of GI bleeding with abnormal clotting tests in CLD cases were weakly positive for PT (RR = 1.02; 95% CI, 0.49-2.10), negative for aPTT (RR=0.83; 95% CI, 0.47-1.45), strongly positive for decreased platelet counts (RR = 1.96; 95% CI, 1.08-3.56) and also for decreased fibrinogen level (RR = 1.47; 95% CI, 0.64-3.35). Conclusion: Coagulation abnormalities were profound in CLD. Decrease platelet counts and fibrinogen levels were related with GI bleeding but PT and aPTT were not significantly related with GI bleeding in patients with chronic liver disease. Nevertheless, these parameters (PT and aPTT) were still used as prognostic markers. (author)

[en] After a quantitative measure of factor II and VII adsorbed at the platelet surface, the authors study the comportment of these factors opposite to: un-numerous washes, irradiations of 0 to 70000 R. It appears that these factors are not solidly fixed at the platelet as: 10 washes are sufficient to get rid of each of the 2 factors; 1500 R overthrow factor VII; 40000 R are necessary to suppress F II. [en] Background. Distal migration is one of the common complications after insertion of a covered metallic stent. Stent repositioning or removal is not always possible in every patient. Therefore, trimming using an argon plasma coagulator (APC) may be a good alternative method to solve this problem. Methods. Metallic stent trimming by APC was performed in 2 patients with biliary Wallstent migration and in another patient with esophageal Ultraflex stent migration. The power setting was 60-100 watts with an argon flow of 0.8 l/min. Observations. The procedure was successfully performed and all distal parts of the stents were removed. No significant collateral damage to the nearby mucosa was observed. Conclusions. In a patient with a distally migrated metallic stent, trimming of the stent is possible by means of an APC. This new method may be applicable to other sites of metallic stent migration

[en] An immunoradiometric assay of procoagulant factor VIII antigen was developed using a human antibody to the procoagulant activity of factor VIII (VIII:C). The assay measures specifically the antigen related to factor VIII procoagulant activity (VIIICAG) both in plasma and in serum. VIII:C and VIIICAG were measured in a group of healthy individuals and in a group of haemophilia A patients. In haemophilia A at least four groups can be recognised on the basis of the presence or absence of VIII:C and VIIICAG and the VIII:C/VIIICAG ratio. (Auth.)

[en] In the present work isoelectric focusing (IEF) in polyacrylamide gels (PAG) with immobilized pH gradient (IPG) and two-dimensional gel electrophoresis (2-DE) were used to characterize the microheterogeneity of the glycoproteins Factor IX (FIX) and Antithrombin III (ATIII) derived from human plasma. A pattern consisting of 6 spots for each of the two isoforms ATIII-alpha and ATIII-beta was observed with isoelectric points from 5.08 to 5.36. FIX was separated into 4 isoforms with pI values ranging from 4.15 to 4.40. The role of high concentrations of chaotropics and detergents is discussed as well as the formation of dimers and trimers dependent on the sample preparation. Results obtained by 2-DE were compared with those of capillary zone electrophoresis (CZE) and capillary IEF, covering also aspects of isoform quantitation. Capillary reversed phase liquid chromatography (RPLC) and CZE coupled to matrix-assisted laser desorption/ionization (MALDI) time-of-flight (TOF) mass spectrometry (MS) by a fractionating robot were used for structural characterization of ATIII and FIX based on analysis of tryptic peptides. The coverage of the amino acid sequence amounts 73 % for ATIII and 71 % for FIX. Furthermore, tryptic peptides with oxidized methionine residues and gamma-carboxylation could be detected. The role of sample composition and background electrolyte (BGE) chosen with respect to on-column sample preconcentration in CZE is discussed. (author)
