[en] Purpose: A comparative treatment planning study was performed to compare volumetric-modulated arc (RA) to conventional intensity modulated (IMRT) for involved-field (IFRT) and involved-node (INRT) radiotherapy for Hodgkin lymphoma (HL). Methods and Materials: Plans for 10 early-stage HL female patients were computed for RA and IMRT. First, the planning target volume (PTV) coverage and organs at risk (OAR) dose deposition was assessed between the two modalities. Second, the OAR (lung, breast, heart, thyroid, and submandibular gland) dose-volume histograms were computed and compared for IFRT and INRT, respectively. Results: For IFRT and INRT, PTV coverage was equally homogeneous with both RA and IMRT. By and large, the OAR irradiation with IFRT planning was not significantly different between RA and IMRT. For INRT, doses computed for RA were, however, usually lower than those with IMRT, particularly so for the lung, breast, and thyroid. Regardless of RA and IMRT modalities, a significant 20-50% decrease of the OAR computed mean doses was observed with INRT when compared with IFRT (Breast DMean 1.5 ± 1.1 vs. 2.6 ± 1.7 Gy, p < 0.01 and 1.6 ± 1.1 vs. 2.9 ± 1.9 Gy, p < 0.01 for RA and IMRT, respectively). Conclusions: RA and IMRT results in similar level of dose homogeneity. With INRT but not IFRT planning, the computed doses to the PTV and OAR were usually higher and lower with RA when compared to IMRT. Regardless of the treatment modality, INRT when compared with IFRT planning led to a significant decrease in OAR doses, particularly so for the breast and heart.

[en] In 1980, based on experimental and clinical data, a protocol was developed at the Institut Gustave-Roussy, alternating 8 monthly courses of chemotherapy and 2, then 3, radiotherapy sequences (15 Gy in 6 fractions and 10 days to the initially involved areas), for early stage unfavourable histology non-Hodgkin lymphomas (NHL). The results are updated for 55 selected patients presenting with bulky stage I and II NHL, intermediate and high grade according to the Working Formulation. Five-year overall survival rate was 69 per cent and freedom from progression 68 per cent. Early haematologic and digestive tolerance was satisfactory, probably because a 10-15 day interval was respected between chemotherapy and radiotherapy and vice versa. No late toxicity was detected in 39 patients who presented with head and neck localizations; xerostomia was found to be only mild and transient. All patients given mediastinal irradiation experience radiological media-stinitis but functional impairment was usually moderate. One of the 4 patients who received 3x15 Gy radiotherapy courses to part of the abdomen, died of small bowel obstruction and perforation. The study demonstrated the feasibility of an alternated schedule of chemotherapy and radiotherapy, with satisfactory results in terms of long-term survival. However, the few late complications which were detected after irradiation of the abdomen or of the late thorax led to an alteration of the initial scheme when these volumes are to be treated. [en] Primary breast lymphoma (PBL) is a rare lymphoproliferative disease. More than 80 % cases show B-cell origin. Diffuse large B-cell lymphoma is most common type. The treatment depends on the histologic type of the non-Hodgkin lymphoma. Mastectomy and axillary dissection are poor factor for overall survival and lymphoma specific survival according to the retrospective analyses and therefore surgery should be limited to the biopsy. Cases demonstrate different histologic types of the PBL with different surgery and chemotherapy approach. (author)

[en] High-dose therapy with autologous stem cell transplantation is one of standard therapies in the management of malignant lymphomas, especially relapsed Hodgkin lymphoma and aggressive lymphomas. The role of allogeneic transplantation is less clear due to high treatment-related mortality. Current position of high-dose therapy with stem cell transplantation will have to be re-defined because of availability of new drugs, like unconjugated and conjugated monoclonal antibodies. (author)
