AbstractAbstract
[en] Since 1970 renal transplantation is done our country and almost from its beginning were radioisotopes techniques used for its evaluation. The most frequent fatal complication is acute rejection and for its monitoring renal renography is routinely used. Early detection of the rejection onset is achieved by this technique, obtained results up to 72 hours earlier than clinical biochemistry investigations. Other early complications such as acute tubular necrosis, cyclosporin nephrotoxicity, vascular thrombosis and urinary obstruction are also diagnosed by these techniques. Nationwide spread of them is possible and several efforts have been done to achieve this goal

AbstractAbstract
[en] Plane gammagraphy employing 99Tcm-DMSA (dimercaptosuccinic acid) is widely used for the study of various renal disorders which affect the parenchyma, whereas the single photon emission computed tomography (SPECT) technique is not. Generally speaking, the different inclination and orientation of the longitudinal axes of both kidneys in patients results in a high level of variability in the detection of the various types of complaint and can lead to an ambiguous diagnosis. In order to avoid this, some software has been developed at the Centro de Investigaciones Clinicas to automate the processing of renal tomography studies with a view to making it as independent of the observer as possible. It is based on an algorithm which performs a series of reorientations of each kidney separately yielding a final image. This facilitates greater objectivity and convenience in the analysis of each case. In this way, a standardization of the procedure is achieved, which means that serial developmental studies of patients can be performed with greater consistency. (author). 6 refs, 4 figs

[en] A formulation of freeze-dried kit with ior t3 (anti-CD3) monoclonal antibody (MoAb) is presented. Once labelled with 99mTc purity higher than 95.0 % was reached. HPLC showed a low proportion of aggregates and fragments. A clinical trial is made with patients without clinical signs of renal graft rejection by means of 99mTc -ior t3. Preliminary results (n=5) showed a rapid and active uptake in the kidney when an acute graft rejection is present. Acute tubular necrosis and other renal complications showed lower concentrations of the radiopharmaceuticals

[en] For the study of different renal affections with repercussion in the parenchyma is widely used the plane gammagraphy wit 99m Tc-Dmsa though not in the same way the SPECT technique. In general, the different inclination and orientation of the longitudinal axes of both kidneys in the patients entail aid to high variability in the detection of the different types of defects which leads to a possible mistaken diagnostic. With a view to this,it was developed in our centre a methodology for the automated reorientation of the different renal volumes obtained by SPECT and its posterior processing, obtaining as result a software with a high grade of independence from the operator. In this way, it is obtained a procedure standardization and so it let us with major rigor to realize evolutive studies of the patients. (Author)

AbstractAbstract
[en] Diagnosis of renal graft acute rejection is made mainly by clinical signs, humoral and functional changes, and is confirmed by histopathological studies. Many efforts have been made to find a radiopharmaceutical for early diagnosis of renal graft rejection. 99mTc-ior t3 monoclonal antibody was evaluated as a radiotracer in renal graft acute rejection. Schwarz's method was used with different molar relations 2ME:IgG and different tin-chelates were explored. The selected formulation was studied by chromatography and challenge studies. Once proved the quality of the radiopharmaceutical a freeze-dried Kit was performed. Its radiochemical purity and stability was studied for several months. FPLC and immunoreactivity studies were included. The sterility and apirogenicity was certified by the Center of Research and Development of Drugs. A clinical trial was started with patients who have received a renal graft eleven days before. An amount of 1.1 GBq of 99mTc-ior t3 MoAb was administered and planar scintigraphic images were recorded. A molar relation of 2000:1 (2ME:IgG) with 30 min produced the best reduction of the MoAb. From the different tin-chelates, the tin-pyrophosphate produced the highest radiochemical purities. The MoAb freeze dried Kit contains 1.0 mg of protein and 14 ug of Sn+2, this quantities guaranteed a shelf life of 6 months. The quality controls and challenge studies showed purity higher than 95.0% and a per cent of dissociation close to 20% was seen with DTPA, 80% with HSA and 48% with Cys. Our preliminary immunoscintigraphic results showed a good correlation among scintigraphy, a high rate of counts Renal Graft/Opposite side and the cause of graft loss. The small number of cases does not permit to make a definite conclusion. However, in some clinical situations the use of 99mTc-ior t3 MoAb could help as an early and specific non-invasive diagnosis of renal graft rejection. (author)
