[en] Kidneys are the main route of elimination for many antineoplastic drugs and their metabolites. The kidney dysfunction may lead to the drug cumulation in organism with the resulting increased systemic toxicity. A lot of used cytostatics requires a dose modification at different levels of renal insufficiency. Due to the lack of data from clinical trials, the limiting of systemic toxicity is difficult especially in patients with severe renal impairment or patients undergoing chronic hemodialysis. The following article is focused on the preventive strategies dealing with recommended dosing modification of various antineoplastic agents in patients with renal insufficiency. (author)

[en] In clinical practice we often meet with impairment of elimination organs in cancer patients before or during the administration of systemic antineoplastic therapy. Given limited available data from clinical trials, the treatment of this patients group resembles a serious problem. It is mainly on issues of higher risk of toxicity, but also possible changes in effectiveness of administered drugs. The article is devoted to the problems of administration of systemic chemotherapy in patients with liver dysfunction. (author)

AbstractAbstract
[en] Colorectal carcinoma (CRC) is the third most common type of cancer and the second most leading cause of death. The combination of cytostatics (irinotecan, oxaliplatin, and fluorouracil) and monoclonal antibodies (bevacizumab, cetuximab or panitumumab) represents the standard first-line treatment of metastatic CRC (mCRC). The objective of this case report is to mediate an experience with a mCRC patient treated with bevacizumab, oxaliplatin, and fluorouracil (5-FU) with a partial response after three and six months. Neurotoxicity and neutropenia were noticed side effects. Oxaliplatin was excluded due to neurotoxicity and 5-FU dose reduced due to neutropenia Venous thrombosis in the right axillary vein developed during bevacizumab therapy. The patient is being continued with bevacizumab and 5-FU after nine-month treatment when the disease is stable. (author)

AbstractAbstract
[en] The epithelial thymic neoplasms involve thymoma and thymic carcinoma. They are the most frequent thymic malignancies in adults. The incidence of more indolent thymoma is higher, whereas thymic carcinoma with more aggressive behaviour and worse prognosis is diagnosed less commonly. It is essential to distinguish other mediastinal tumors within differential diagnosis. The correct determination of clinical stage based predominantly on the presence of tumor invasivity is an important factor for the selection of adequate treatment. The exact determination of the histopathologic subtype may be difficult. The major prognostic factors include clinical stage, histopathologic subtype and the possibility of radical surgical resection. The therapy may be multimodal, depending on the clinical stage and patient characteristics. Except for surgery, radiotherapy and chemotherapy, more drugs from the group of targeted therapies have been evaluated. (author)
