[en] Scintigraphy of the pancreas is important because of a lack of simple internal and x-ray pancreas diagnostic examination methods, non-burdening to the patient, yet providing sufficient evidence. We conceived a double isotope subtraction method aimed at widespread application; financially, it should be within the range even of smaller nuclear medicine departments. A scanner is combined with double impulse processing and a subtraction unit (Picker Dualscanner) and an adapted x-ray unit with the x-ray tube aimed at the scan-field. Commercial sup(Se-75)selenium methionine is used for pancreas imagining. sup(TC-99m)colloidal sulphur is used as a liver indicator. After barium-brei application orally, an x-ray is taken of the gastro-intestinal tract, so as to be able to delineate the pancreas from other epigastric organs also able to accumulate methionine. The subtraction photoscan is then inscribed on this pre-exposed film without any shift of the patient. It is also possible to use two parallel films (x-ray/photoscan) and then to superposition them

[en] The radioimmunological determination of triiodothyronine and thyroid-stimulating hormone (TSH) provides two new methods for functional diagnoses of the thyroid. Changes and disturbances of the control mechanism had been difficult to detect with conventional methods. The measurement of the basal TSH rate has further therapeutic and diagnostic consequences such as, e.g., the determination of growth stages of strumae or preclinical hypothyreosis on the basis of increased basal TSH values. The results of the TRH test enable a more exact clarification. Substitution therapy should take into account the rate of the remaining basal TSH secretion as well as the clinical picture. The determination of the serum T3 concentration enables the diagnosis of isolated triiodothyronine hyperthyreosis. Continuous control of the thyrostatic therapy by T3 and TSH determination helps to prevent a condition in which hyperthyreosis persists even though all the normal parameters indicate a euthyroid function. The development of a preclinical hypothyreosis can also be detected early. The increased basal TSH secretion after strumectomy is a further proof of the urgent need for consequent prophylaxis of relapses (orig./AK)

Mit der radioimmunologischen Bestimmung von Trijodthyronin und thyreoideastimulierendem Hormon (TSH) stehen 2 neue Methoden der Schilddruesenfunktionsdiagnostik zur Verfuegung. Aenderungen und Stoerungen des Reglermechanismus waren mit den bisherigen Methoden schwer erfassbar. Die Messung der basalen TSH-Rate bringt zusaetzliche therapeutische und diagnostische Konsequenzen. So lassen sich augrund erhoehter basaler TSH-Werte Wachstumsphasen von Strumen und praeklinische Hypothyreosen erkennen. Die genauere Klaerung ist durch den Ausfall des TRH-Testes moeglich. Bei der Substitutionstherapie des Myxoedems sollte man sich neben dem klischen Bild auch nach der Hoehe der verbliebenen basalen TSH-Sekretion richten. Die Bestimmung der T3-Konzentration im Serum ermoeglicht die Diagnose der isolierten Trijodthyroninhyperthyreose. Die laufende Kontrolle der thyreostatischen Therapie durch T3- und TSH-Bestimmungen kann verhindern, dass die hyperthyreote Stoffwechsellage persistiert, obwohl die ueblichen Parameter eine euthyreote Funktion vortaeuschen. Auch die Entwicklung einer praeklinischen Hypothyreose kann fruehzeitig erkannt werden. Die erhoehte basale TSH-Sekretion nach Strumaresektionen beweist erneut die Notwendigkeit der konsequenten Rezidivprophylaxe. (orig.)Original Title

[en] The clinical signs and symptoms of Paget's disease are extremly variable. About 20% of cases are asymptomatic, and the diagnosis is made accidentally in the course of X-ray or radioisotope studies. Usually, however, pain is present as an early symptom. It may manifest itself as headache or be of rheumatoid, degenernative or inflammatory nature. In the present study, 99m-Tc-MDP bone scans of 18 patients with different diagnoses or suspected diagnoses referred for radio isotope evaluation were analyzed. After identifying the are as of increased uptake (133 areas of abnormal nuclide uptake), the findings from the scans were compared with those obtained on conventional radiology. On the basis of typical localizations or localizing patterns an attempt was made to establish type-specific criteria for the diagnosis of Paget's disease and its differential diagnosis versus other conditions. Scintigraphy is a highly sensitive tool for the early detection and identification of the extent of pathological bone processes. In addition, it is superior to conventional radiological methods, as the radiation dose rendered to the patient is low and the entire skeletal system can be adequately visualized. (Author)

[en] The investigation of 4 commercially available T4-RIA-Kits with 350 patient sera have shown that: The separation of hypo-, eu- and hyperthyroid values is easier with kit a) and d) than with b) and c), though all kits give acceptable values under routine conditions. For the determination of the 'normal range' not less than 100 patient sera are necessary. The time needed for 100 determinations is 2 to 3 hr. A general recommendation for one of the tested T4-RIA-Kits cannot be made since the choice depends also on local circumstances. (orig.)

Fuer die Normalbereichsbestimmung sollten mindestens 100 Patientenseren benutzt werden. Die benoetigte Zeit pro Ansatz fuer 100 Einzelbestimmungen belaeuft sich auf etwa 2 bis 3 Std. Eine generelle Empfehlung fuer einen der hier getesteten T4-RIA-Kits kann nicht gegeben werden.
