[en] The use of Tc-99m DTPA renal flow imaging has increased in importance since the introduction of catheter angioplasty. The high diagnostic accuracy and non-invasive technique as compared to digital subtraction angiography have done much to enhance its usage in the evaluation of renal functional status. This paper reports Tc-99m DTPA renal flow imaging findings before and after angioplasty in a patient with bilateral renal artery stenosis. The slight decrease in Tc-99m DTPA renal flow on the kidney before angioplasty converted to an increase in flow on the right kidney after angioplasty. Post procedural blood flow was much higher on the right, where stenosis was previously more severe. It is hypothesized that the right kidney was protected from elevated aterial pressure by its 90% arterial stenosis while the left kidney was exposed to elevated systemic blood pressure. Because of vascular damage to the unprotected left kidney, post-angioplasty blood flow was less in the left kidney than in the ischemic but protected right kidney. (orig.)

Die Darstellung der renalen Perfusion mit Tc-99m-DTPA hat seit Einfuehrung der Katheter-Angioplastik an Bedeutung zugenommen. Die grosse diagnostische Genauigkeit und die Nicht-Invasivitaet der Technik im Vergleich zur digitalen Subtraktions-Angiographie haben dazu beigetragen, den Stellenwert der Technik in der Beurteilung des renalen Funktions-Status zu steigern. An dieser Stelle wird ueber Befunde der renalen Fuss-Darstellung vor und nach Angioplastik bei einem Patienten mit bilateraler Nierenarterien-Stenose berichtet. Der leicht erniedrigte Tc-99m-DTPA Fluss der Niere vor Angioplastik wandelte sich zu einer Zunahme des Flusses der rechten Niere nach Angioplastik. Der Blutfluss war rechts, wo die vormalige Stenose ausgepraegter war, viel hoeher. Es wird vermutet, dass die rechte Niere durch die 90%-ige Stenose vor dem erhoehten arteriellen Druck geschuetzt war, waehrend die linke Niere dem erhoehten systemischen arteriellen Druck ausgesetzt war. Durch die vaskulaeren Schaeden der dem hohen Druck ausgesetzten linken Niere war der post-angioplastische Blut-Fluss hier geringer als in der vormals ischaemischen, aber nicht dem hohen Druck ausgesetzten, rechten Niere. AbstractAbstract
[en] We present markedly decreased radioactivity in the right on a 99mTc-pertechnetate thyroid image in a patient with secondary lymphoma of the thyroid. At autopsy, the right lobe of the thyroid was replaced by lymphoma tissue. Markedly decreased radioactivity on the images may be explained by lymphoma cell infiltration or replacement of thyroid tissue with a resultant attenuation effect as well as interference of the trapping mechanism of the thyroid folicular cell. While cold areas in a radionuclide scintigram may present difficulties in interpretation, a high index of secondary lymphoma suspicion should be made by a combination of the scintigraphic findings and history of malignant lymphoma. AbstractAbstract
[en] Cholescintigraphy of cholelithiasis has been reported as a defect or photon deficient area in the gallbladder. We present the case of a patient with gallstones whose cholescintigraphic study showed a sequential evaluation of the scan appearance from a defect, to a septation, a hole, and finally a filled-in appearance. Cholescintigrams and histopathologic findings after cholecystectomy of another 22 patients with cholelithiasis were also retrospectively reviewed. The results of the study concluded that: (1) cholelithiasis is rarely demonstrated by cholescintigraphy and (2) nonvisualization of the gallbladder may be due to acute or chronic cholecystitis associated with cholelithiasis. AbstractAbstract
[en] To evaluate gastric emptying in patients with bulimia, 20 patients (all women, ranging in age from 12 to 49 years) with upper gastrointestinal symptoms ingested 150-200 μCi99m Tc-triethelene tetraamine polysterene resin in cereal and had scintigraphy in the supine position. Data were accumulated at 5 min intervals to determine the gastric emptying time (GET). The results showed that the gastric emptying time was prolonged in 12 patients and decreased in 8. All 12 patients with prolonged emptying time were given 10 mg metoclopramide intravenously; 9 of these had a good response and 3 had no response. Although all patients had subjective symptoms of gastric dysfunction, the results indicate that about 60% had delayed and 40% had rapid gastric emptying. The findings of two extremes of gastric emptying time remain to be explained, however, this enables (Albibi and McCullum 1983) objective documentation of gastric emptying as this technique (American Psychiatric Association 1980) can separate those patients with rapid GET from those with prolonged GET, who might benefit from metoclopramide. [en] Pulmonary edema induced by injections of oleic acid was demonstrated using 111In-chloride lung/heart image in five anesthetized dogs. The anesthetized dogs were positioned under a gamma camera interfaced to a computer. After 111In-chloride IV injections, dynamic data were recorded at 1 frame/min before and after induced pulmonary edema. The computer generated the curve of the lung-to-heart activity ratio; the ratio rose from 0.5-0.6 at baseline to 0.85-1.5 at th end of the study in the dogs. At the end of each study, the removed lungs demonstrated edematous fluid, and a few red blood cells were seen in the alveoli. The results indicating pulmoinary edema exhibited in the 111In-chloride lung-to-heart ratio obtained froma computerized gamma camera were compatable with the pathological results. Since 111In-chloride instantly binds to plasma transferrin, an excellent intravascular imaging agent, leakage of this tracer into alveoli indicates loss of membranous wall competence. Pulmonary edema, as in the adult respiratory distress syndrome, is sometimes difficult to diagnose; and this 111In-chloride lung-to-heart imaging technique may be potentially useful. (orig.)

AbstractAbstract
[en] Meningioma is one of the neoplasms in which there may be extraosseous localization of bone-seeking radiopharmaceuticals. Tumor calcification, calvarial erosion, and the formation of reactive bone have been proposed as the cause of this abnormal tracer localization. We present a patient with a frontal meningioma that was evaluated using sup(99m)Tc-methylene-diphosphonate bone scintigraphy, head computed tomography, and skull radiography; the homogeneous density seen in the radiographic studies corresponding to the area of bone-seeking-agent localization shown in the scintigram. At autopsy, bony tissue and a few psammoma bodies were found in the meningioma, and apparently accounted for the bone-tracer localization. There was no calvarial erosion and no formation of reactive bone. If skull-radiographic studies show a homogeneous, radio-opaque lesion with no reactive changes in the adjacent skull, a meningioma showing a localization of a bone-seeking radiopharmaceutical may be predicted to have bone-tissue formation with or without psammoma bodies.
