AbstractAbstract
[en] The accuracy of large volume selective arteriophlebography in demonstrating the main renal veins in 28 patients with renal carcinoma is presented. The diagnosis was shown to be correct at operation in 96% of examinations. It is suggested that the technique is valuable and safe and should form part of the routine pre-operative assessment of patients with renal carcinoma. (author)

[en] The use of the EMI CT 5005 scanner in patients with renal calculus disease is described. This investigation was found to be a useful aid in the accurate localisation of calculi within the renal collecting system prior to surgery. (author)

AbstractAbstract
[en] The results of prospective experimental and clinical studies into the use of radionuclides in the evaluation of urinary flow rates and residual urine volume estimations are presented. Studies were made following renography using 123I-Hippuran or sup(99m)Tc-DTPA. The results indicate that the radionuclide estimation of flow rates is accurate, but complex and time-consuming compared with other available methods. The calculation of residual urine is simple and accurate where residual volumes are not extremely large. The opportunity for such lower tract studies after renography should not be missed where clinically appropriate, and the combined upper and lower tract studies provide a useful quantitative urological profile. [en] In patients treated for malignant testicular tumours without clinical and/or radiological signs of tumour, the following serum hormone levels were found. In 15% of the patients there was a slight to moderate rise in luteinising hormone (LH) levels (up to 6μg/1) due to increased pituitary gonadotrophin production; this was particularly evident soon after radiotherapy/chemotherapy. In 50% of the patients there was a slight to marked increase to follicle stimulating hormone (FSH) levels (up to 11μg/1), especially after radiotherapy/chemotherapy. Serum testosterone levels were in the low range (up to 20 nmol/1) in the majority of the hemicastrated patients regardless of previous treatment. A slight to moderate rise in serum oestradiol-17β and serum prolactin levels was noted. During combination chemotherapy with vincristine, Adriamycin D, cyclophosphamide, actinomycin D and medroxyprogesterone acetate the serum testosterone levels were extremely low (below 6 nmol/1) with LH and FSH levels within the normal range. The decrease in testosterone levels was reversible after completion of the combination chemotherapy. (author)

AbstractAbstract
[en] Sixteen patients with severe intractable haemorrhagic cystitis following radiotherapy, two of them with bleeding tumours, were treated surgically by cutaneous ureterostomy with or without contralateral nephrectomy. They had undergone various forms of conservative treatment and were treated by surgery only when conservative therapy had failed. In these poor risk patients a cutaneous ureterostomy was employed as a lesser procedure than an ileal loop. Of the 16 who underwent urinary diversion 11 were completely free of haemorrhage and three continued to have slight intermittent haematuria. (author)

[en] Further improvements were made in the techniques of human prostatic lymphoscintigraphy, allowing better anatomical localisation of the areas of uptake of activity. A single median injection into the capsule of sup(99m)Tc labelled antimony sulphide colloid was found to give as good imaging as 2 injections on either side of the midline. By placing markers on the umbilicus, pubic symphysis and both anterior superior iliac spines, a 'pelvic grid' could be superimposed on the antero-posterior view. Further help with accurate localisation may be attained by taking 3 views; anterio-posterior, postero-anterior and lateral. In 9 instances, prostatic injection was performed without any sedation or anaesthesia. The resulting scintigrams were indistinguishable in quality from those of anaesthetised patients. (author)

AbstractAbstract
[en] Computed tomography (CT scanning) was undertaken in 51 patients with bladder tumours, 15 of whom subsequently had pathological. Concordance between the CT and pathological staging (80%) was as good as that between clinical and pathological staging (73%). Most of the inaccuracies of clinical staging were underestimations of the extent of muscle invasion, whilst the inaccuracy of CT involved overstaging. The principal areas where CT might improve the accuracy of clinical staging are in patients with invasive tumours who after transurethral resection have no mass palpable on bimanual examination and in patients following radiotherapy who might be candidates for salvage cystectomy but in whom assessment of the spread of tumour is difficult because of post-irradiation fibrosis. Data from a much larger series are required to ascertain whether the additional information provided by CT will produce any improvement in patient management. (author)

AbstractAbstract
[en] A urographic survey of 250 patients being followed for bladder cancer revealed 3 unsuspected ureteric tumours and 18 cases of ureteric obstruction. The tumours occurred in patients always affected by bladder recurrences and the obstructions were largely due to treatment of paraureteric bladder tumours by cystodiathermy or radiotherapy. Regular follow-up intravenous urography is therefore unnecessary except in patients with frequent recurrences, but we advocate radionuclide renography as an essential, inexpensive screening test to rule out ureteric obstruction following the treatment of paraureteric tumours. [en] Deconvolution analysis of different regions of interest on gamma camera renography enables obstructive lesions causing impairment of nephron function to be distinguished from obstructive lesions in which parenchymal function is unimpaired. Quantitation of isotope transit time through the parenchyma is a reliable method of diagnosing upper urinary tract obstruction. (author)
