[en] A 70 year old male presented with right lower quadrant pain and anorexia for about 72 hours prior to hospital admission. He underwent laparotomy with impression of acute appendicitis. The operative finding was an inflamed tumoral appendix without apraclonidine base involvement , and appendectomy was done. The patient had an uneventful post-op course. Due to the pathology report of prostatic carcinoma he underwent cystoscopy and biopsy of prostate on the 4th post-op day. Histopathology of the appendix with prostatic origin. Histopathology of the prostatic specimen was consistent with high grade prostatic adenocarcinoma

AbstractAbstract
[en] Prostate-specific antigen is a 33-KDa serine protease that is produced predominantly by prostate epithelium. However, it has been shown that about 30-40% of female breast tumors produce prostate-specific antigen and its production is associated with the presence of estrogen and progesterone receptors. We have now developed a new tissue culture system to study prostate-specific antigen production in breast cancer and its association with prognostic factors such as progesterone receptor and c-erbB-2. For this purpose we investigated the ability of prostate-specific antigen production in five different cell lines, including two breast cancer cell lines, Sk-Br-3 and MDA-MB-453. The prostate-specific antigen in tissue culture supernatant and cytoplasm of the Sk-Br-3 cell line was detected by western blotting and immunoperoxidase, respectively. Furthermore, we found lower expression of c-erbB-2 in Sk-Br-3 than non-prostate-specific antigen producer breast cancer cell line, MDA-MB-453. Progesterone receptor was expressed by both prostate-specific antigen-positive and -negative cell lines and only the intensity of staining and the number of positive cells in Sk-Br-3 population was higher than MDA-MB-453. According to our findings prostate-specific antigen can be considered as a good prognostic factor in breast cancer and we suggest that these two cell lines are a good in vitro model to study the relationship of different breast cancer prognostic factors and their regulations

[en] Previous studies have shown that injection of iodized oil in pregnant women can be used as a prophylactic strategy for iodine deficiency disorders and may improve the growth indices of their offspring. Since administration of pharmacological doses of iodine may lead occasionally to large goiter and rarely to hypothyroidism, in the present study the thyroid function tests of neonates and infants born to women who had received 480 mg iodized oil intramuscularly during pregnancy were assessed and compared to those of a control group. Off 277 cord blood samples obtained from Mazandaran and Khohkiluyeh-Boyerahmad provinces, 125 made up the case (injected) and 152 the control (non-injected) group. Of 1026 blood samples of the neonates and infants from Mazandaran province. 544 made up the case and 482 the control group. Serum T4, T3 and TSH concentrations were measured with RIA kits. in the cord blood samples, mean serum T4 in cases who had received iodized oil was lower than that of the control group:140± 32 vs. 149± 33 nmol/L, respectively; p<0.03. T3 and TSH were not however different. In the neonates and infants, T4 and T3 concentrations were significantly higher in the case than control group:178± 40 vs. 168± and 3.5±0,02 nmol/L, respectively, both p<0.001. In contrast, TSH concentrations were lower in the experimental group in coparison to control:2.6± 2.2 vs. 3.1± 2.9 mU/L, respectively; p<0.001. Increased T3 and decreased TSH were seen in infants of mothers who were injected in the second and third trimesters of pregnancy. Injection of iodized oil in pregnant women does not cause hypothyroidism in the offsprings, however it does cause a transient increase in serum thyroid hormones and a decrease in TSH concentrations

[en] The aim of this study was to analyse polar lipids of candida species isolated from Ahwas (Iran) by fast Atom bombardment mass spectrometry . Nine isolates of Candida Sp. were identified by growth at 45digc, production of chlamydoconidia on cornmeal agar, colonial colour on CHROMagar Candida, germ tube production and ID 32 C kits. Then polar lipids were extracted from freeze-dried cultures and analysed using Fast Atom Bombardment Mass Spectrometry. The most intense carboxylate and phospholipid molecular species anions were of m/z 281 (C18:1) and m/z 515 (PA 23:2). However, the most intense carboxylate and phospholipid analogues in Candida Parapsilosis were 292 (Un) and 555 (PA 26:3), which differed from other yeasts. Isolates were grouped by single linkage clustering based on correlation coefficient for strain pairs calculated with carboxylate and phospholipid molecular species distributions. Fast Atom Bombardment Mass Spectrometry can differentiate the C. albicans based on analysis of polar lipid distributions.These findings support that differentiation between C. albicans and other species is possible based on polar lipids

[en] Radiation-related wounds challenge surgeons in all disciplines of surgery. Wound-healing complications are commonplace, and solutions for reconstruction are limited. Muscle and musculocutaneous flaps have improved this situation. But the question is, does previous radiation of the muscle to be transposed affect the outcome? 143 consecutive previously irradiated patients treated with muscle or musculocutaneous flaps composed the group under consideration: these 143 patients had 206 muscles transposed. The overall complication rate for muscle transposition to close a radiated wound was 20 percent. Of the 143 patients who received radiation, 62 patients had the muscle transposed for wound closure from the primary field of radiation. 81 patients were closed with non-irradiated muscle. When the transposed muscle had been radiated, the complication rate was 29.6 percent; in 14.3 percent, the entire muscle underwent necrosis, requiring total removal and a second tissue transposition from a non-irradiated source to achieve closure. The subgroup using non-irradiate muscle had a complication rate of 12.2 percent; 1 patient in this group had complete flap necrosis requiring a second tissue transposition. No postoperative deaths were encountered. The experience in our department reveals that non-irradiated muscle is the MJIRI, Vol. 15, No.4, 189-193, 2002

[en] Errors and variations in interpretation can happen in clinical imaging. Few studies have examined the biased effect of clinical information on reporting of brain CT scans. In a blinded double crossover design, we studied whether three radiologists were biased by clinical information when making CT scan diagnosis of the brain. Three consultant radiologists in three rounds with at least a one month interval assessed 100 consecutive cases of brain CT scan. In the first round, clinical information was not available and 100 films without clinical information were given to radiologists. In the second round, the same 100 films were given and true clinical information was available. In the third round, the same 100 films were given and false clinical information was allocated. In 180 cases (60%) the evaluation resulted in the same diagnosis on all three occasions (95% confidence interval (CI): 54.5, 65.5), whereas 120(40%; 95% CI:34.5, 45.5) sets were evaluated differently. 48 cases (16%; 95% CI:11.9,20.1) had discordant evaluation with true and 33 (11%; 95% CI:7.5, 14.5) with false clinical information. Discordance without and with true and false clinical information was 39 (13%; 95% CI:9.2, 16.8). Correct clinical information improves the brain CT report, while the report became less accurate false clinical information was allocated. These results indicate that radiologists are biased by clinical information when reporting brian CT scans
