[Article review] EUS-Guided LAMS for Pancreatic Fluid Collections - What the Latest Study Says : A systematic review & meta-analysis compared three types of lumen-apposing metal stents (LAMS)—Axios, Nagi, and Spaxus—for EUS-guided drainage of pancreatic fluid collections (PFCs). Key Findings (Pooled Results from 37 Studies, 3,074 Patients): ✅ Technical Success: Axios (97.7%), Nagi (96.9%), Spaxus (98.2%) ✅ Clinical Success: Axios (90.9%), Nagi (88.5%), Spaxus (93.5%) ⚠️ Adverse Events: Highest in Axios (20.4%), lowest in Spaxus (7.6%) ⚠️ Stent Migration: Highest in Nagi (7.8%), lowest in Spaxus (0.9%) ⚠️ Bleeding: Highest in Axios (7.0%), lowest in Spaxus (1.8%) Takeaway: All three LAMS show high success rates, but Spaxus had the lowest migration & complication rates. 𝗘𝗨𝗦-𝗴𝘂𝗶𝗱𝗲𝗱 𝗥𝗮𝗱𝗶𝗼𝗳𝗿𝗲𝗾𝘂𝗲𝗻𝗰𝘆 𝗔𝗯𝗹𝗮𝘁𝗶𝗼𝗻 65-yo patient affected by pancreatic metastasis of renal cell carcinoma referred to our centre Fondazione Poliambulanza Istituto Ospedaliero Multispecialistico The endoscopic treatment consisted of endoscopic ultrasound-guided radiofrequency ablation (EUS-RFA). Preliminary EUS + Sonovue (CH-EUS) demonstrated a 9x6 mm hyper vascular lesion in the head of the pancreas. Distance from MPD and duodenal wall was > 5 mm and 2 mm respectively. A dedicated 19G RFA needle was used to ablate the lesion. Post treatment CH-EUS demonstrated a dramatic reduction in vascularity of the lesion. Follow up CT scan is scheduled in 1 month. Expanding indications for EUS-RFA. We recently took care of a young female patient suffering from a pathology-proven lymph node metastasis from G2 pancreatic neuroendocrine neoplasm (pNEN) diagnosed 18 months after distal pancreatectomy. CT-scan and Ga-Dotatoc PET-CT confirmed the single 10 mm celiac lymph node localization. After a multidisciplinary discussion with oncologists and surgeons, we decided to perform EUS-guided radiofrequency ablation of the malignant lymph node. The procedure was performed in a supine position under general anesthesia, using a UCT-180 linear-array Olympus EMEA echoendoscope coupled with EU-ME3 ultrasound processor. A 10-mm EUS-RFA probe from TaeWoong Medical EUSRA was used coupled with the VIVA Combo RF Generator with 50 Watt power setting. Three 30-second applications were necessary to reach a complete ablation, as confirmed by post-EUS-RFA contrast-enhanced EUS. No procedural or post-procedural adverse event occurred and the patient was discharged from the Hospital of the Azienda Ausl Di Imola the day after the procedure.
