A 68-year-old man was admitted to another hospital for paraneoplastic encephalitis. CT thorax showed a small lung nodule and a 2.3cm subcarinal lymph node. I performed linear EBUS and got good core biopsy and aspirate for cell block. Didn’t need ROSE. Didn’t do cryobiopsy. It was enough to diagnose small cell carcinoma on the next day. ‘Simplicity brings more happiness than complexity’. Thomson Hospital Kota Damansara

79 y/o male who came initially to an outside center with acute right sided body weakness, and slurred speech. IV Tenecteplase (TNK) administered at referring center. Upon arrival to our center, stroke MRI showed persistent basilar artery occlusion (BAO), with an acute pontine infarct. Vessel wall imaging (VWI) showed features of possible intracranial atherosclerotic disease (ICAD). Went to the angiography suite, suction x 1 attempted - no clots retrieved. Balloon angioplasty was then done with improved critical stenosis/occlusion. Patient remains well post angioplasty, with aggressive medical management 🙏🏻
