Happitech: Transforming AF ablation aftercare with remote patient monitoring St Antonius Hospital in the Netherlands is leading the way in virtual cardiac care, leveraging Happitech’s hardware-free heart rhythm monitoring via Luscii to improve post-atrial fibrillation (AF) ablation aftercare. Key Achievements: - 1000 patients treated, with 35 new patients/month joining the pathway. - Reduced cardiology team workload & increased AF ablation capacity. - Improved patient outcomes and quality of life. - Patients empowered to manage their heart health remotely. - A scalable model for future healthcare transformation. How it works: - Patients track their heart rhythm at home using a smartphone - eliminating the need for routine ECGs at the hospital. - Specialist e-nurses monitor readings via the Luscii dashboard, ensuring continuous care. - If no arrhythmia is detected, patients graduate from the program after 12 months - reducing unnecessary visits while ensuring safety.
