This initiative, launched by the Local Health Unit ULS de Coimbra, in collaboration with the Municipality of Ansião and the Hospital de Avelar - Fundação de Nossa Senhora da Guia, represents a significant milestone in healthcare innovation. 🏥 The new facility is a cornerstone of the "S@úde +Perto" initiative, a telemonitoring project now entering its second phase. The program aims to deliver innovative and integrated care to 6,600 patients across the ULS de Coimbra region, ensuring seamless coordination between primary care and hospital-level follow-up. At the core of this program is the Hope Care, SA HCAlert platform, which serves as the technological backbone for remote patient monitoring. The CAC in Avelar was specifically established to enhance access to healthcare for residents of five municipalities: Alvaiázere, Ansião, Camara Municipal De Castanheira De Pera, Figueiró dos Vinhos and Pedrógão Grande. By offering high-quality, proximity-based care services, the initiative addresses a critical need for local healthcare solutions. This collaboration between ULS de Coimbra, Hospital de Avelar - Fundação de Nossa Senhora da Guia and local authorities highlights the transformative power of partnerships in providing essential healthcare to underserved communities.
