Today, we're spotlighting a key figure in our efforts to enhance mobility care. Dr Dominic Chen leads the SKH Mobility Clinic, specialising in Rehabilitation Medicine. The clinic focuses on helping individuals with mobility challenges, including those recovering from strokes or undergoing amputee rehabilitation. Dr Chen shares his approach: "In Rehabilitation Medicine, we evaluate mobility issues from various perspectives. Our team, including physiotherapists, works to understand each patient's condition. We often encounter cases related to age-associated muscle loss, joint problems affecting movement, and nerve issues in the legs due to diabetes." Dr Chen recalls a notable case: "We treated a young patient who lost fingers and toes due to severe infection. This patient faced significant mobility challenges and had to use artificial limbs. Our team worked with the patient to relearn how to walk using these prosthetics. This case highlighted the complex nature of mobility rehabilitation and the importance of timely, specialised care." Many patients with advanced liver cirrhosis face a challenging journey—enduring high symptom burden, frequent hospitalisations, and reduced quality of life. Yet, palliative care is often only offered at the terminal stage, when it’s considered "medically futile." 🩺💔 Read Asst Prof Marianne’s inspiring story that highlights the need for earlier, supportive care through SKH’s Palliate End-Stage and Advanced Cirrhotics Early (PEACE programme) that transforms patient care. “I created the PEACE programme that integrates palliative care earlier in liver disease treatment to improve patients’ quality of life. Led by a liver specialist with a multidisciplinary team, it’s a liver supportive care programme focused on holistic care including symptom control, psychosocial care, caregiver support and advanced care planning. One case I remember clearly on the need for this approach was a young patient with liver cirrhosis who had complications like liver cancer, water retention, bleeding, and jaundice. Despite his health struggles, he continued working to support his elderly mother. His liver tumour and bleeding were treated and transplant was being discussed. But because he was young, seemingly “stable” despite advanced disease and hopeful for a cure, the topic of end-of-life care – like legacy planning and care for his mother – was never brought up. It wasn’t until a terminal event that he received supportive care just days before his passing and we learnt he’d been living with pain and anxiety all along. Among the participants was Mr Goh, who reflected on his own journey: “I’ve been proactive in monitoring my prostate health through screenings for the past decade. Despite being active and healthy, I discovered an enlarged prostate that required surgery to prevent complications. Early detection and regular screening are crucial for timely medical intervention. Together, through this event, we’ve raised awareness, sparked conversations, and encouraged men to prioritise their health.” Thank you to everyone who joined us and championed this cause.
