Diagnosed with end-stage renal failure, the late Mr Koh found solace and care in our palliative ward, surrounded by a compassionate team who supported him and his family through his final days. 💚 His sister Cynthia shared, “He struggled to believe anyone cared for him, but at SLH, he found peace.” Through patience, respect, and unwavering kindness, our team eased his discomfort, fostered reconciliation, and ensured a dignified farewell. To our palliative care team, thank you for embodying care beyond physical healing, touching hearts when it matters most. 🙏️ Read the full story: https://lnkd.in/gyMJz6aU #ServingLovingHealing #PalliativeCare #TransformingLives #CompassionInCare

This World Swallowing Day, we recognise our dedicated Speech Therapists, who launched their very first mocktail push-cart initiative for patients in the wards! 🍹 With five unique flavors to choose from, patients enjoyed delicious zero-sugar thickened drinks, which supports hydration for those with swallowing difficulties, also known as dysphagia. Stella, a patient referred to SLH for rehabilitation after knee-cap surgery, chose the "Sunset Mojito" and added: "Thank you for making our rehabilitation journey here joyful, and for always encouraging and reassuring us." 💚 #ServingLovingHealing #SpeechTherapy #Rehab

Last week, we had the privilege of hosting Minister of State, Mr Murali Pillai, and our valued partners to St Luke's Hospital for the Bukit Batok Health and Social Care Fellowship networking lunch! This meaningful gathering provided an excellent opportunity to reconnect with friends from the healthcare and community care sectors and explore new possibilities for collaboration. During the session, Mr Murali reminded us of the importance of working together and keeping an eye out for residents who may have fallen through the gaps. Together with our partners, we remain committed to delivering quality care and meeting the evolving needs of our community ❤️ #ServingLovingHealing #Partnerships #Community

Thank you, Channel 8 前线追踪, for spotlighting Working Out to Wellness (WOW), part of SLH's Western Silvercare (WSC) #programme! Designed for #seniors aged 60 and above, WOW focuses on improving strength, endurance and balance through exercises developed by SLH #Physiotherapists. These exercises not only improve mobility but also empower seniors to perform daily tasks with greater ease and confidence. “Many seniors feel stuck at home due to mobility challenges,” explained Raymond, Senior Exercise Physiologist at SLH. “Through this programme, we’ve seen improvements in their mood, independence, and even the formation of new friendships during the sessions.” Beyond exercise, WSC also offers #healthscreening and monitoring through our Community Health Post and health talks led by our medical team to meet the #community’s needs. 🎥 Watch the feature: https://lnkd.in/gNCK8vWJ #ServingLovingHealing #WOW #WesternSilvercare

At SLH's 17th Wound Conference, we were thrilled to bring together passionate professionals committed to advancing wound care. Themed 'Collaborative Wound Care Partnership,' the conference served as a vibrant hub for learning and collaboration. Experts from SLH, Ng Teng Fong General Hospital (JurongHealth Campus), Tan Tock Seng Hospital, and Singapore General Hospital shared insights on key topics, including vascular wounds, chronic diabetic foot ulcers, and the role of nutrition in wound care. SLH's Community Wound Centre highlighted its impactful nurse-led, community-based care model, while a panel discussion explored patient-centred interdisciplinary wound management. Through interactive workshops by Intellimed Healthcare and Coloplast, participants had fun applying advanced wound care techniques on apples to better understand biofilm management techniques and learn innovative cutting guides for optimal wound management. As part of our commitment to addressing pressure injuries, we partnered with Coloplast to spotlight The Triangle of Wound Assessment 2024, with winners getting $3,000 educational credits for SLH's wound care courses. A heartfelt thank you to all participants, partners, and sponsors for joining this vibrant community striving to elevate wound care. We look forward to seeing you next year for another inspiring chapter of learning, sharing, and innovation! ✨ #ServingLovingHealing #TransformingCommunityCare #SLHWoundConference2024 #WoundCare
