And just in case the people only appeared in the settings menu after getting to the scene they appear in, I tried that, too. And they do not appear in the settings menu in the "patched" English release even after the scene in which they first appear.

Maybe there are cut things or censorship; I haven't gone through the whole game. But at least for this one thing that has you concerned, it's not censored.
Are you playing with the update patch they released last year?
Because at the time this topic was made, the background was definitely censored (and progress within the game had nothing to do with that -- I thoroughly tested this with both the original Japanese release and this one).
But I have not bothered to return to it since the update patch was released.
As I stated in early posts, night time was unique to the non-ero patch Steam build. Daytime was for the ero-patch build and it had no people. The Japanese release had people even on a brand new install with no save data.
For this and sequel if possible.
How much censorship is with patch?
Anything else that might be relevant (like censorship but it is/it's not a big deal)
Game has mosaic censorship but it's not that bad (still drop in quality)
Otherwise, this game is more about it's story and characters than sex.
Idea being, there are differences from the original Japanese version, but nothing to prevent enjoying the story
BTW, i found out a remaster is planned. No clue if it will be Japanese only, but i sure wished i knew before buying this game (assuming remaster version is a separably sold game with more story)
Hope this info will help anyone else to not hesitate to pick it up and read (again, assuming you do 100%, since one or two routes only = "good story" only, with many funny moments).
