    Advanced technology does not automatically equate to real world clinical utility. I have been pitched the following 15 times (I counted): "We will use AI to analyse multi-omics data (pathology, biochemistry, EHR data, radiology) to determine the: risk of developing / optimal treatment / adverse events of [insert disease here]" This sounds pretty good right? Except that in the real clinical settings, it surfaces up 3 major questions: 1. 𝐖𝐡𝐨 𝐢𝐬 𝐠𝐨𝐢𝐧𝐠 𝐭𝐨 𝐛𝐞 𝐢𝐧𝐩𝐮𝐭𝐭𝐢𝐧𝐠 𝐭𝐡𝐞 𝐝𝐚𝐭𝐚? Most similar algorithms are created using datasets that have been cleaned and structured. In the real world setting, these data sits in 6-7 different systems. Unless the algorithm is automatically integrated with all of them, It will be the clinicians who have to input the data themselves. Meaning a significant increase in workload and liability. Now this is fine if there is real clinical benefit, Which brings us to our second question. 2. 𝐖𝐡𝐚𝐭 𝐚𝐫𝐞 𝐰𝐞 𝐬𝐮𝐩𝐩𝐨𝐬𝐞𝐝 𝐭𝐨 𝐝𝐨 𝐰𝐢𝐭𝐡 𝐭𝐡𝐞 𝐝𝐚𝐭𝐚 𝐨𝐮𝐭𝐩𝐮𝐭? Let's take an example of an algorithm that predicts the risk of developing lung cancer in 10 years time. Imagine if a patient were to be determined as high risk by the AI. 🤔 What is the GP supposed to do with this result? - Should he refer to his respiratory colleagues based on a proprietary result from an external vendor? - Are we going to have to investigate the patient now with CT scans every single year? - If the CT scans come up negative (which will be likely the case), do we then do random lung biopsies to find cancer? OR do we do nothing because there is nothing we can do, and just leave the patient in worry and suspense for 10 years? 3. 𝐖𝐡𝐲 𝐬𝐡𝐨𝐮𝐥𝐝 𝐜𝐥𝐢𝐧𝐢𝐜𝐢𝐚𝐧𝐬 𝐭𝐫𝐮𝐬𝐭 𝐭𝐡𝐞 𝐨𝐮𝐭𝐩𝐮𝐭 𝐨𝐟 𝐲𝐨𝐮𝐫 𝐀𝐈? Practice changing innovation requires game changing evidence. To convince the entire medical fraternity to adopt any solution, particularly one that is consequential: You will need evidence. Not just a paper or two. Prospective studies done in real clinical settings on a sufficiently large sample size. Without demonstrating that the output of your algorithm is indeed credible in the real life setting, there is slim to zero chance anyone will take it seriously. _____________________________________ So, why is clinical utility so important to start-up founders anyway? Because no clinical utility = no commercial viability. If you are not able to demonstrate that your tool can improve patient clinical outcomes, how are you going to sell to health systems exactly? This is not an anti-technology post. My hope is for founders building similar tools above to consider clearer how their ultimate product is going to look, feel and work in the clinical setting. Simply put, assets are anything physical or digital that helps your company communicate your value to others. It could be:  📽️ Videos  🌐 Website  📚 Brochures  🧑🏫 Sales decks  📰 Publications  ⭐ Testimonials Whatever helps you sell without you needing to be there, is an asset. _____________________________ #HealthTech notoriously has a low production rate of assets. And those who do, produce assets that are not aligned with what the #NHS or #clinicians want. Leading to poor sales and engagement.     A healthtech company without clinicians is simply a tech company. But attracting (and retaining) the best clinical talent is a challenge. Today, I will be breaking down 5 key principles that any healthtech founder will need to recruit the best and the brightest clinical talent. ________________________________________ 1. 𝐊𝐞𝐞𝐩 𝐲𝐨𝐮𝐫 "𝐚𝐬𝐤" 𝐜𝐥𝐞𝐚𝐫 𝐚𝐧𝐝 𝐬𝐢𝐦𝐩𝐥𝐞. The clearer you can state what you need from them, the better. State their likely role and responsibilities. Tell them what they won't need to do.  Clarify how many hours you expect. Clinicians are time poor.  You need to make it easy for them to assess whether they can commit or not. It is ill-advised to mislead them at first - stating that the role is light, But turn back on your word and start to ask them to do more later. Scope creep leads to a lot of dissatisfaction. 2. 𝐁𝐞 𝐬𝐢𝐧𝐜𝐞𝐫𝐞 𝐚𝐧𝐝 𝐚𝐮𝐭𝐡𝐞𝐧𝐭𝐢𝐜, 𝐬𝐦𝐨𝐤𝐞 𝐚𝐧𝐝 𝐦𝐢𝐫𝐫𝐨𝐫𝐬 𝐰𝐨𝐧'𝐭 𝐥𝐚𝐬𝐭 𝐥𝐨𝐧𝐠. Be clear about your values and mission early on. Some founders try to excite clinicians by stating: - How much funds they have raised - The LOI and traction they have received so far. - What world class accelerators they have been through. But such things ultimately don't mean much. When the going gets tough (and trust me it will) All these novelties will eventually run stale. If both of you believe in the mission from day 1,  They will go the distance. 3. 𝐃𝐨𝐧'𝐭 𝐬𝐞𝐭𝐭𝐥𝐞 𝐭𝐨𝐨 𝐪𝐮𝐢𝐜𝐤𝐥𝐲 𝐨𝐫 𝐞𝐚𝐬𝐢𝐥𝐲. Some founders make the mistake of pouncing on the very first clinician that says yes. Unless you are very lucky, you will be better served talking to a few more clinicians. Remember, they have to be: A good fit for your team's culture Free enough to commit the required hours A genuine expert that can push the needle forwards It is a bad idea to offer up precious equity to someone you barely just met. 4. 𝐀 𝐜𝐥𝐢𝐧𝐢𝐜𝐢𝐚𝐧 𝐫𝐞𝐚𝐜𝐡𝐞𝐬 𝐨𝐮𝐭 𝐭𝐨 𝐚 𝐜𝐥𝐢𝐧𝐢𝐜𝐢𝐚𝐧 𝐞𝐚𝐬𝐢𝐞𝐫. The secret to building a solid clinician circle is to get another clinician to do it. It is much easier for someone with a familiar background to reach out. You come from a much better position of trust,   And having a clinician who trusts you first sends a good signal. 5. 𝐅𝐢𝐧𝐝 𝐞𝐪𝐮𝐢𝐭𝐚𝐛𝐥𝐞 𝐰𝐚𝐲𝐬 𝐭𝐨 𝐰𝐨𝐫𝐤 𝐭𝐨𝐠𝐞𝐭𝐡𝐞𝐫. If you want to attract and retain the very best talent, Think seriously about what you can offer to keep them. Talented clinicians that have the unique skillset and mindset to work with startups are a rare bunch. We have seen startups offer reimbursement in many creative ways.     Clinical adoption is the holy grail of HealthTech success. 😫 But so many companies can't seem to find it. I have produced this carousel as part of a presentation I am doing this week together with Barry Shrier as part of the GIANT Health Event. The truth is, most healthtech innovation don't get used. 👇 Often it stems from a: Lack of evidence Lack of awareness Lack of clinical need Lack of engagement Lack of change management But there are also remedies to the above and I have listed 7 key recommendation for HealthTech companies in this carousel. Simple as it sounds, there are great challenges getting entire populations to adhere to all 6 principles at scale.     Clinical evidence separates the wheat from the chaff. This is true for any #healthcare innovation. But let's be honest - clinical evidence generation can be tricky. 👇 To generate high quality evidence, one needs to: 1. Understand what and how to evaluate products. 2. Define clearly your value proposition and intended use. 3. Have a good grasp of regulatory requirements. 4. Know what type of evidence #NHS procurements requires. 5. Learn what defines good clinical research design. 6. Learn what evidence VCs are looking for if fundraising. 7. Have hands on knowledge on how to navigate academia. 😫 It is tough to find the knowledge for all the above.     What are HealthTech startups looking for when hiring clinicians? We have facilitated over 100 clinician consulting jobs so far, And here are 5 things we notice startups ask for: 1. 𝐓𝐡𝐞 𝐫𝐢𝐠𝐡𝐭 𝐜𝐥𝐢𝐧𝐢𝐜𝐚𝐥 𝐞𝐱𝐩𝐞𝐫𝐢𝐞𝐧𝐜𝐞 𝐢𝐧 𝐭𝐡𝐞 𝐫𝐢𝐠𝐡𝐭 𝐬𝐩𝐞𝐜𝐢𝐚𝐥𝐭𝐲. - Clinical experience is table stakes. It is the first starting point. - Unsurprisingly, we get a lot of interests for GPs as they are quite versatile. 2. 𝐇𝐚𝐯𝐢𝐧𝐠 𝐧𝐢𝐜𝐡𝐞 𝐞𝐱𝐩𝐞𝐫𝐢𝐞𝐧𝐜𝐞 𝐡𝐞𝐥𝐩𝐬 𝐜𝐥𝐢𝐧𝐢𝐜𝐢𝐚𝐧𝐬 𝐬𝐭𝐚𝐧𝐝 𝐨𝐮𝐭. - Startups often require something specific that isn't easy to find. - I.E. A respiratory physician with experience setting up remote patient monitoring and virtual wards. 3. 𝐂𝐥𝐢𝐧𝐢𝐜𝐢𝐚𝐧𝐬 𝐰𝐡𝐨 𝐡𝐚𝐯𝐞 𝐚 𝐟𝐥𝐞𝐱𝐢𝐛𝐥𝐞 𝐦𝐢𝐧𝐝𝐬𝐞𝐭 𝐚𝐧𝐝 𝐫𝐢𝐬𝐤 𝐦𝐞𝐧𝐭𝐚𝐥𝐢𝐭𝐲. - The startups we work with often end up choosing a candidate that meshes well with the initial scrappy culture. - We see that this matter MORE than years of experience or seniority. 4. 𝐂𝐥𝐢𝐧𝐢𝐜𝐢𝐚𝐧𝐬 𝐰𝐢𝐭𝐡 𝐬𝐤𝐢𝐥𝐥𝐬𝐞𝐭𝐬 𝐭𝐡𝐚𝐭 𝐜𝐚𝐧 𝐬𝐨𝐥𝐯𝐞 𝐚 𝐛𝐮𝐬𝐢𝐧𝐞𝐬𝐬 𝐩𝐫𝐨𝐛𝐥𝐞𝐦. - An obvious example will be clinicians who are clinical safety officers who can assist in their DCB 0129 filings. - Having an academic skillset (designing clinical studies and writing) can really help too. 5. 𝐂𝐥𝐢𝐧𝐢𝐜𝐢𝐚𝐧𝐬 𝐰𝐡𝐨 𝐚𝐜𝐭𝐮𝐚𝐥𝐥𝐲 𝐡𝐚𝐯𝐞 𝐭𝐢𝐦𝐞 𝐭𝐨 𝐝𝐨 𝐭𝐡𝐞 𝐰𝐨𝐫𝐤 𝐫𝐞𝐪𝐮𝐢𝐫𝐞𝐝. - Sounds silly, but for more hands on projects - startups require clinicians that can spare a day a week to help with project development. - Not all clinicians have this kind of flexibility. _____________________________ But if you are a clinicians seeking out opportunities in HealthTech, You dont just need to show that you have: 1. The right clinical experience. 2. Specific helpful niche knowledge. 3. A innovative and collaborative mindset. 4. A broad skillset allowing you to contribute. 5. Time availability to commit to longer projects. You need to be able to communicate the above clearly too. 𝐒𝐡𝐨𝐰𝐢𝐧𝐠 𝐢𝐬 𝐛𝐞𝐭𝐭𝐞𝐫 𝐭𝐡𝐚𝐧 𝐭𝐞𝐥𝐥𝐢𝐧𝐠.
