Anterior knee pain in runners. Definition: “Runners knee” or anterior knee pain is the most common knee injury in runners especially after half marathons and marathons. Anterior knee pain is characterized by pain that worsens when flexing the knee, such as when running, squatting, or descending stairs. With research showing Anterior knee pain was the most frequent new injury in the half-marathon runners after the competition (1), it is important for runners of any level to take preventative steps leading up to any running prevent to best avoid this condition. Both amateur and professional athletes are at risk for this kind of syndrome. AKP contributes to 5.4% of all sports injuries in athletes,2 with long-distance runners being the main group of athletes at high risk for developing AKP. Risk Factors; 1. Over running and not having an appropriate progressive program in the lead up to the event. 2. Biomechanical Factors; Running technique, muscle weakness, lack of flexibility, concerns with foot posture and pelvic stability 3. Previous knee injuries; This feeds into potentially altered biomechanics 4. Lifestyle factors; Weight, diet, exercise levels Structures Involved; There are multiple structures which can cause anterior knee pain, which tends to be an umbrella term to describe pain in the front of the knee. These structures can include the patella tendon, quadriceps tendon, fat pad, kneecap positioning and many more. Treatments; This would be customised based on assessment findings, common approaches include. 1. Tailored running program under the guidance of Physiotherapist and running coach, this incorporating short runs, long runs, interval runs and recovery days. 2. Gait analysis provided by Physiotherapist to assist with any biomechanical issues. 3. Graded strength and stretch program to assist running program and improve performance 4. Appropriate footwear 5.
