  • Attain and colleagues at Dartford, Gravesham & Swanley Health & Care Partnership and Dartford & Gravesham NHS Trust have been shortlisted for an HSJ Partnership award.   Attain and NHS colleagues have worked together to design and mobilise the investment of some of the hospital’s budget in community services.   Investment is focused on third sector dementia support in ED, community teams in the hospital and Hospital at Home and virtual ward and care coordination capacity in the community. The goal is to improve care, reduce the number of admissions and reduce length of stay. We are excited to see the results of these innovative initiatives as they are delivered through the winter.

  • As the winter months set in, there is greater pressure on ensuring timely discharges from hospitals. At Attain, we recently established a programme of initiatives at place-level to unblock the process of discharge and provide greater grip and control. This programme contained the following five strands: ⭐ Understand the full picture - We established a single place-based performance report encompassing health and care activity to provide one source of truth with the metrics that matter for joint boards, reducing duplicitous work. ⭐ At-a-glance visibility of the out of hospital services/pathways running “hot” - We deployed a community escalation tool entirely focused on out of hospital services to complement the national OPEL tool escalation tool and aid place–level decision making when the acute flows are under pressure. ⭐ Drive productivity through digital adoption - We aim to support ROI from digital investment by maximising the digital features used, in turn helping to release staff time spent on manually creating reports. ⭐ Using data to inform investment decisions - We provided independent analysis of the capacity and demand across all resources, supporting discharge to assess pathways by providing empirical and unbiased recommendations for future investment needs. ⭐ Digital approach to support operational grip and control - Within a gap in the market, we created a three-stage strategy to establish a transfer of hub dashboards, co-produced with operational staff from health and care teams. It will provide live views of remaining capacity, activity, delays/queues, and demand across health and care funded teams providing admission avoidance and discharge support that will support good operational decision making. To find out how we can work with you to understand the issues you face and establish a programme to respond, please get in touch with us at contacts@attain.co.uk

Project management is crucial for the NHS to ensure the efficient and effective delivery of patient care. Optimised resource allocation, controlled costs, and managed risks all contribute to sound project management. These are essential to helping organisations across the NHS achieve better results that ultimately lead to improved patient outcomes and operational efficiency. At Attain, we understand there isn’t a one-size-fits-all approach to project management. That’s why we have created a comprehensive framework and tool kit for delivering effective project management that is flexible and adaptable to organisational needs.
