The Future NHS Award is for successfully trialling and embedding change to make better use of innovations, research data and digital technology, to provide more convenient access to services and information for patients and improve support for staff. MyRenalCare revolutionises the management of chronic kidney disease by putting patients in the driving seat of their care. It caters for all stages of kidney failure and types of renal replacement therapy. Patients self-monitor their condition against personalised targets set by their clinical team. They capture their own clinical observations, symptoms and can access their blood results. They can also proactively contact their clinical team when needed and access unique animated education content helping them to better self-manage their condition. Clinicians can access this data and deliver 100% digital ‘virtual’ consultations without the need for a phone call, video call or face to face consultation. Clinicians send their patients reminders to do a blood test and are alerted when the results are ready. Clinicians review the data and share any recommended changes to their patients treatment strategy on the platform. They can remotely monitor their patients condition at any time and are alerted if their condition exceeds any set parameters. The additional and continuous patient captured data enables clinicians to make better decisions. MyRenalCare significantly increases capacity and reduces costs for healthcare providers, reduces the environmental impact of managing chronic kidney disease and improves health outcomes for patients.

Great day yesterday at DigitalHealth.London's South London networking day: Great to hear; a) Lord Darzi's recommendations are already being baked into ICB priorities 2) ICB's see innovation as being critical in addressing the current issues within the NHS, especially in creating additional capacity in the face of growing demand. Patient empowerment and self monitoring along with ambient AI were called out is key game changers. Thank you for sharing Michael Bell! 3) Pitches from fellow Digital Health London accelerator innovators - always inspiring to hear the amazing new solutions coming down the pipeline and in many cases already being used and transforming healthcare for the better Great to meet, chat with and learn from; a) Inspiring leaders across South London including; Toby Garrood, Michael Wright, Matea Deliu MD, PhD, MRCGP and hear about their priorities, what types of innovation they are most excited about, and what would need to be true for innovation to be adopted. b) Likeminded innovators - their key successes, pitfalls to avoid, top tips and just having a good laugh over a glass of wine. 🍷 To add to the occasion, the event was hosted at the Oval Cricket grounds members club - such a beautiful building with so much amazing history! Today we're in Kazakhstan! Sharing our lessons learnt, and ideas for solving the big 3 issues with how CKD is managed today. There are 3 key problems with how CKD is managed globally: #1 Lack of Nephrology capacity in tertiary care - resulting in waiting lists and delayed care. Firstly we've been invited to share the experience and learnings from using MyRenalCare to #transform the management of individuals living with #ckd at Portsmouth Hospitals University NHS Trust. So far MyRenalCare in the UK has enabled >3000 virtual appointments with independent research showing this increases renal outpatient clinic capacity by >30%. We also shared our new projects which are underway to help solve the other 2 big issues: #2 The majority of people with CKD are undiagnosed (including the UK!) MyRenalCare has teamed up with Nova Biomedical to enable point of care testing the in community with instant results for creatinine and eGFR. This enables targeted screening in the community and is a highly scalable solution for diagnosing CKD early in high risk populations. #3 Under-treatment of early stage CKD. This is a tragedy as there are now drugs that significantly slow the progression of CKD, helping to significantly reduce the number of people who reach end stage renal failure and require dialysis or a transplant. MyRenalCare enables a "specialist digital renal team" to manage early stage CKD remotely, allowing for rapid up titration of medication. This enables individuals with early stage CKD to be optimally treated early and is highly scalable. We are actively looking for new partners globally!
