Today we publish the first interview in our new series ‘Speaking up for patient safety’, produced in collaboration with Peter Duffy, retired urology consultant, NHS whistleblower and Chair of the Healthcare Working Group at WhistleblowersUK: https://lnkd.in/eFRjmarc Our first guest is Helené Donnelly OBE, who spoke up about unsafe care she witnessed while working as a nurse at Mid Staffordshire NHS Foundation Trust. Helené contributed as a witness to the inquiry led by Sir Robert Francis KC into failings at the trust and was also an advisor in the Freedom to Speak up Review in 2015, where she called for the creation Freedom to Speak Up Guardians in the NHS. In the interview, Helené speaks to Peter and Helen Hughes about why she decided to raise concerns about the quality of nursing care at Stafford Hospital A&E and describes the bullying and threats she received from other staff as a result. She discusses the barriers that still prevent staff speaking up today and what can be done to create a more open and responsive culture in teams and organisations. Psychological safety is when someone feels they are safe to speak up with ideas, questions, concerns or mistakes without fear of being punished or humiliated. In healthcare, developing a culture of psychological safety is essential to ensuring patient safety. It helps to create and maintain an environment where patient safety issues can be raised, discussed and resolved. A psychologically safe environment supports incidents of avoidable harm being responded to with empathy, respect, rigour and action for improvement. NHS England has recently commenced a review of two digital clinical safety standards: ▶️ DCB0129: This defines clinical risk management requirements for manufacturers of health IT systems. ▶️ DCB0160: This defines clinical risk management requirements for organisations that use health IT systems. The review will begin with a set of online focus groups canvassing opinion on the current sufficiency of the standard DCB0129. As part of this, NHS England are actively seeking input from a range of stakeholders, including NHS professionals, IT manufacturers and those involved in digital risk assessment.
