𝗧𝗵𝗮𝘁’𝘀 𝗮 𝘄𝗿𝗮𝗽 🎬️ Last week, we had a great day of filming for some brand new how-to videos for our 𝗧𝗛𝗘𝗥𝗠𝗔𝗥𝗠𝗢𝗨𝗥 𝗠𝗲𝗱𝗶𝗰𝗮𝗹 𝗕𝗹𝗮𝗻𝗸𝗲𝘁𝘀 - why do we need how-to videos for something so obvious you ask? Because warming patients 𝗣𝗥𝗘 and 𝗣𝗢𝗦𝗧 surgery is still getting overlooked! This is despite 𝗡𝗜𝗖𝗘 𝗴𝘂𝗶𝗱𝗲𝗹𝗶𝗻𝗲𝘀 warning that this can lead to serious post operative complications... We hear time and time again that current warming solutions are too complicated to use on the ward. 𝗧𝗛𝗘𝗥𝗠𝗔𝗥𝗠𝗢𝗨𝗥 are here to change that. Clinicians need simple solutions that work. Our passive warming blankets can be used 𝗕𝗘𝗙𝗢𝗥𝗘, 𝗗𝗨𝗥𝗜𝗡𝗚 𝗮𝗻𝗱 𝗔𝗙𝗧𝗘𝗥 surgery. Vapour Barrier is it a myth ? What is it ? How can we achieve it ? When should we attempt it ? How can we monitor & treat a patient in a vapour barrier envelope ? All these questions came to light following this article shared by Jamie Pattison ( Team lead of North Umberland SAR ) https://lnkd.in/eUdtimZv After allot of discussion between Jamie , Gemma Aldridge Ben Cooper Dave Gregory and other members of World Extreme Medicine | B Corp Certified faculty. We came up with this (as described by Luca Alfatti FRGS, FEWM, MCPara ) Evidence based on common sense summary; Transportation of Patient and Vapour Barrier 1- Options A) True Vapour Barrier - Vapour Pressure Equilibrium . A vapour barrier closest to the patient . Allowing volume of air saturated with vapor , achieving vapour pressure equilibrium. Limiting evaporated heat loss. B) A barrier to prevent loss in vapour - Burrito wrap C) Prioritise Shelter - Removal of Wet clothes - Insulation . 2- Following factors will point in direction of whether to aim for ; A) For primary retrieval, environmental temperature excessively low , conflict situation. Where layers are available and accessible , rapid removal to protective shelter is priority. B) A situation where you are modifying and compromising, essentially to rapidly transport the patient to secondary care. Where time is in favour for not repeatedly opening the envelope to assess and manage the patient. C) Where time is prolonged ( approx more than 30 minutes) or circumstances dictate frequent exposure of patient resulting in frequent vapour loss and progressive uncontrolled hypothermia. 3) Layers A) Casualty wet clothes - Absorbent with impermeable outer layer* ( taped/ strapped for snug fit around casualty, to decrease air space to minimum )- Insulation blanket - External Heat Source ( cannot be placed adjacent to any plastic impermeable outer sheet of absorbent layer as it will damage it ) - Wind waterproof layer. * This was used in the article- https://lnkd.in/ey_wGajR or presume THERMARMOUR blanket could be used ? B) Casualty wet clothes - Burrito Wrap - External Heat Source - Internal Vapour Barrier - Insulation- Outer Vapour Barrier . OR Blizzard Protection Trauma Blanket with Central Velcro OR THERMARMOUR Insulated Stretcher. C) Shelter - Sleeping mat ( possibly Inflatable) - Absorbent layer below casualty (will need frequent changing) - Casualty- Removal of wet clothes ( including underwear and socks) (will need frequent drying) - External Heat source ( over dry clothes or over blanket) - Insulation Layer - Water proof layer . OR TSG Associates LLP Xtact SR System Focus in this PFC situation is - I ) Control Wetness 2) External Heat 3) Insulation 4) Segmental Exposure for ease of access for treatment and monitoring Conclusion - There is no particular right or wrong, it's doing the best for the patient with the kit you have, in the time frame and space you have.

Thank you to the lovely people who met with me at Birmingham Women's and Children's NHS Foundation Trust last week 🏥 It was humbling to meet clinicians specialising in corrective surgery for spinal deformity amongst other challenging childhood conditions, and it was evident how much they care for their patients’ wellbeing 🩵 Birmingham children’s hospital will be trialling a version of our ponchos for pre-warming patients so we’re looking forward to hearing how they get on 🙏🏻
