🏆 60% drop in delayed discharges: Read about how our award-nominated partnership is improving patient flow in Hull and East Riding of Yorkshire Following our exciting HSJ Partnership Awards 2025 shortlist announcement, we're proud to share the incredible impact of our work with Hull and East Riding of Yorkshire, nominated for 'Most Effective Contribution to Integrated Health and Care.' Through innovative partnership working, we've achieved remarkable results in transforming discharge processes: - 60% reduction in delayed discharge patients - Hospital stays cut by a third for supported discharge patients - 102% increase in number of patients receiving intermediate care support - Long-term financial benefits through reduced discharge delays and demand for long term care The success came through true collaboration - bringing together Hull University Teaching Hospitals NHS Trust, City Health Care Partnership CIC, East Riding of Yorkshire Council, Hull City Council, and voluntary sector organisations to create an integrated discharge model that puts recovery first. 👉 Read the full case study here: https://lnkd.in/evJZh6Dw #NHS #HealthcareInnovation #IntegratedCare #HSJAwards #PatientCare

I was honoured to be one of the panellists at this discussion - particularly since I got to share it with such brilliant speakers, including my former boss Neil Mathur (He/Him)!
