Prevention should be the prime focus always, Reactive measures are only required when any incident happened, if preventive measures are taken well in advance the reaction might not be required, So HSE professionals should foresee the incidents as well as communication to the organisational level is also as important as identification or foreseeing…

To start, It is important to review both 'lagging indicators' as well as 'leading indicators'. Looking at loss history, near miss "learning events", and blending this data with personal workplace inspections and worker interviews along with inspection checklist usage is a solid starting point. The employees often know much more about the involved processes and hazards than many safety professionals, so don't overlook their perspectives. They can be one of your most powerful hazard identification sources and workplace safety allies.

Are nearly 100 percent of your staff reporting and/or tagging near miss or unsafe conditions/broken equipment at least once a year? You want everyone focused on their environment, constantly scanning it for possible hazards, and communicating those hazards to make unsafe or abnormal conditions visible to all. If people are used to just walking around a wet spot on the floor instead of reporting and remediating it, it won’t be long before you have an accident. This is what I suggested to improve HID process. 1. Create Behavioral Base Safety Register for all staffs - both good & bad. 2. Create staffs recognition process - motivation 3. Effective data capturing and analyzing will give you ideas of which areas you are lacking and that requires improvement. It is fundamental and will assist you to guide, promote good safety culture and communicate well within the team or as individuals.

To improve hazard identification: 1. Past Incidents:Review past accidents and near-misses. 2. Observations:Regularly inspect the workplace for risks. 3. Employee Input:Involve employees in hazard identification. 4. Industry Standards:Follow industry regulations and standards. 5. Technology:Use sensors and data analytics for real-time insights. 6. Incident Reports:Analyze reports to uncover root causes. 7. Risk Assessments:Use tools to systematically analyze risks. This mix of historical data, observations, employee involvement, regulations, technology, incident analysis, and risk assessments helps in identifying hazards effectively.

In my experience, I deploy the root cause analysis to deep dive into the root causes of incidents with critical incident analysis. Fish bone followed with 5 ways are really helpful to direct us into the recovery actions and what kind latent learnings to be cascaded to corporate to prevent reoccurrence. This creates a learning organization that aims to improve our process safety culture.

In my view, a broad-based approach helps since the aim of hazard identification is to prevent them occurring. I have observed that keeping a journal of hazardous incidents reported globally help to avoid oversight. Again, making a checklist of hazards to watch out for in design and development processes, and ensuring there are available help as well. Review of personnel feedback also provides dependable data in hazard identification

When looking at Data yes the past incidents can help predict future possible occurances however I would encourage industries like private security to look at incident reports, and provide accurate training on Hazard identification and mitigation. Having worked in the private security field I am ashamed to say that the hazards we report on our daily and incident reports get dismissed.

In my extensive experiences in conducting hazards identification& risk assessment duties, it is of utmost importance that these hazards must be quantified. This can be done in the form of scores or rankings. Basics technique such as Likelihood versus Severity must be employed. The end-product is the construction of matrix in the form of RAM - Risk Assessment Matrix (Just Google). Nowadays many companies employ an extended L vs. S scoresheet in order to quantify the hazards identified. Without any scorings/rankings, we can never identified which hazards are catastrophic or not critical. Then, we may apply the 5 Whys to determine the real root causes for factors that lead to these hazards. I discussed these frequently in my Linkedin account.

The data of *unsafe acts & unsafe conditions is most valuable data for identifying the gaps and proximity of incidents. Identified UA/UC* are must analyzed daily basis and real time basis to act proactively to prevent any untoward incident.

Reviewing Job Hazard Analysis in pre-shift safety meetings, be all contractors have a voice to express the hazards in their area. The not-so-well known hazards can be addressed by individual contractors at this time.

You involved the employees in the data collection process so they are mostly on board and now understand what identifying a hazard is and how to break down the steps of a task. How do you ensure they remember the information? You can use identified hazard worksheets to enhance job hazard analysis sheets. Then you can use the job hazard analysis sheets as Job Aids. My JHA's include pictures. Laminate them and place them right at the workstation or in a department binder. Your leadership team and on-the-job trainers now have tools to train, remind, refresh.

By communicating a Job Safety analysis before the start of any task any one performing, spectating , or passing , has the knowledge of all dangers prior to the job . Discussing these dangers can make anyone more aware and able to foresee an incident.

With so many bashing OSHA recordkeeping requirements, I thought I would point out that when PROPERLY kept, that lagging data could be a bright light to follow in the early days of a Hazard ID effort. I have always used the trending from as many years of OSHA injury/illness logs to identify opportunities for JSA/JHAs and just some good old fashion hazard ID exercises. This look also is a great inside look at how mature management is in its Safety Management. If they are mature in their SMS we should find the controls/barriers/safeguard identified in the investigation/RCA still in place and thus the hazard/risk is “controlled”. But if they are immature, we will likely find the hazard present. Also, look at other locations for the risks.

I have seen that risk assessment might be misused by others to justify for business needs over process safety or in some cases they would underestimate the risks involved without involving right experts. Process Safety is everyone responsibility and we all should take an ownership of our daily decisions making sure that these decisions are not causing any harm to people, asset, environment and company reputation.

Safety Kaizen days/week will help to capture such risks in Mass..It is also recommended to control Hazard through design and administravtive controls and then with additional protections from PPE...Henrich theory is must to understand to avoid accidents and eliminate risks making your workplace safer..

Data that should be used to identify hazards at workplace include data received from safety inspection carried out at workplace to identify potential hazrds, safety inspection carried out and other Information received from workers regarding safety at workplace. As this data is received from active monitoring before any accident occurs so can be used to avoid future occurance. Other information includes industry experts and regulator Bodies regarding workplace hazards.
