I begin by diving deep into data collection. It’s tempting to leap straight into fixing things, but real insight comes from stepping back and methodically assembling information. This means engaging in meaningful conversations with your team, delving into relevant documents, and getting a firsthand look at what’s happening on the ground. By piecing together a comprehensive dataset, we’re able to uncover patterns or anomalies that guide us to the root cause. One valuable technique is the '5 Whys' exercise. By the way, this WHY exercise is like peeling an onion; you peel one layer, then the next one, then the next one till you reach the core. Asking as many times WHY as you feel you need, based on your gut. Don't stop with number 5. Ask as many times as you feel you need. This is about treasures lying deep in the ocean. And you do this WHY exercise in writing so you can gain more clarity.

The Five Whys is a great start for root cause analysis. Sometimes, though, we can get bogged down by our own biases and fail to see the true explanation. Then we might solve for something that isn't the true problem. If you find this happening, go back to asking why and consider a totally wild, irrational reason for why something happened. This can jar us out of our biases and help stimulate new ideas for reasons why. What might seem unrealistic at first might actually be the true reason behind the problem. You could also bring in others who have nothing to do with the situation to help give different perspectives. Don't forget that simpler problems might be figured out before the 5th why and more complex ones may take more than 5!

When you open up to asking other people, it is much more likely to reach the desired result, as having other points of view will make it easier to broaden your point of view and reach the result faster and with better quality.

So what to do (assuming you allowed your teammates to contribute)? Bring in external, impartial people to help you examine your ways of thinking, analyzing and doing from new perspectives! I see again and again how much supporting a (leadership) team as a team coach enables them to look at the situation with fresh eyes.
