I find that one of the biggest challenges can be getting managers to fully appreciate their role and responsibility for creating a safe and health system for work, and using risk assessment as a management tool, rather than leaving the process to employees or, worse still, QHSE professionals. Managers should be actively involved in risk assessing the work activities and workplaces they are responsible for.

A key challenge for everyone involved in risk assessment is to avoid making assumptions on individual behavior. In my experience, people are much the same and we should never assume that someone else would behave any differently than we would.

In todays workforce the biggest challenge is simplicity when it comes to reporting and communication. Health and Safety professionals are trained and put through courses to be able to interpret legislative documents. The general populace is not so why would we make our workplace systems that complicated and so full of terminology that is unnecessary. In my experience communication and reports should be as simple and basic as possible to prevent misinterpretation. Pictures can be a marvelous substitute when trying to explain a situation. Remember everyone needs to be involved with the safety process. They won't feel involved if they do not understand.

The whole concept of PDCA was based on the idea that it is better to be approximately right at the start and then to improve through practice. With this philosophy, the biggest constraints seem to be 1) keeping things implemented consistently, and 2) reviewing and improving in a planned way. Far too often, changes to work practices are made in a reactive way without consideration of the risks but, because it worked once, the new work practice becomes the default.

As QSHE professionals, we sometimes don't do ourselves any favors by making the risk assessment process over complicated and focusing on numbers or scales for likelihood, consequence and risk. In many cases it is enough to recognize the hazard for what it is - like falling from a roof, working in an excavation, or using a particular chemical. The processes and working practices required to control these hazards are all well known and don't require some artificial scale to determine how they should be controlled. Let's try to keep it simple.

Como auditora de terceras partes he notado que dos de los mayores desafíos de la evaluación de riesgos de QHSE son: 1. La metodología, en algunas organizaciones muy compleja, en otras con criterios vagos que no aportan utilidad o que solo entiende el responsable del sistema de gestión; y 2. La continuidad, la revisión y el seguimiento es una vez al año; la herramienta no es un documento "vivo". Entre las limitaciones: 1. Visión estrecha de implementación: solo al contexto y/o algunos procesos claves; pero no a la totalidad del sistema de gestión y sus procesos; y 2. En el ámbito de las normas ISO, específicamente ISO 14001 e 45001, las organizaciones aún no identifican riesgos más allá de los impactos ambientales y riesgos laborales.
