Holding a hotwash or after action conference, whichever you call it, is imperative immediately after an incident has come to an end. If you are unable to get everyone together, capture feedback through a questionnaire or poll format document and require responses within a very short time. I've found feedback as a group is helpful in that feedback can generate useful conversation. For times when they're may be sensitive issues or multi-level staff are involved, a feedback form may be a better option where staff may be hesitant to share concerns or comments about a leader. Also, it has proven beneficial to hold the hotwash and follow up with an anonymous questionnaire. Then, you can ask questions pointed to areas not captured.

I’d gather the team for a quick, informal debrief. This would be a safe space for everyone to share initial thoughts and reactions while the experience is fresh. I’d keep it conversational, allowing each team member to voice their perspectives without feeling pressured.
