The number #1 security risk I see in my practice Is that privileges or roles are not revoked when a user changes a location or leaves employment. This also happens with “super user” type authorities and privileges as companies are hesitant to possibly breaking something may depend on a particular super user. However this is a bad practice and opens up a vulnerability. Additionally, Row and Column Access Control (RCAC) can be used to further restrict access based on groups and roles. Furthermore, TSL should be mandatory for all connections to the database. In many instances, this is still optional.
