1. The most important of all is mindset of the people (owner ,director, manager) working ashore in support of vessels operating at sea. They want vessel to run without maintenance as it cost money. For example :- the vessel engine is due for overhaul. What they do, take extension. Suppose in extended time the vessel in mid of voyage had breakdown, will it save money or dispose money. Most of companies ignore OEM recommendations and try to squeeze on maintenance cost. 2nd important factor is Always Yes sir onboard staff. They hide the defects to an extent that only corrective maintenance is possible in case of breakdown. 3rd is fool proof Planned maintenance system. Many more factors are there, linit is only 750 words😄.

The right moment is when a failure is near that is - when the fault starts affecting the machine, but it is still safe to keep the machine running until the time for repair suits the company best (for example during a planned shutdown).
