It’s been my experience that accurate Nursing workload staffing models haven’t succeeded because the traditional time study methods aren’t practical. It requires a functional approach that considers how Nursing Unit staff are capable of organizing their time into “task sets” that enable them to be more efficient. Over 30 years ago we created such a model (using ancient technology) and in 1985 we increased Nursing satisfaction and reduced overtime, and better use of semi-full-time staff, saving over $1M/yr, in 1985$! With today’s smart technology, it seems ripe for an AI project to identify “new” Best Practices, using “Task Sets”, by discipline, level of care, by category of acute care Nursing Service, and the classic Categories of Care.

We developed our own validation by tapping into the great judgement and experience of Unit Managers and Charge Nurses. The Acuity based system spit out the optimum mix and number of staff, by skill, in hours, for an 8 hour shift. The managers came prepared knowing the staff they had, and from their “experience”, the numbers they actually needed. We would use that variance to tweak the standards until we were getting 100% agreements, +/- 4 hours. We also instituted cross service audits to help maintain integrity and transparency, to prohibit ”Acuity creep”.

Validation is key. Be objective, and remember that no one tool is perfect. Conduct pilot studies, perform statistical analyses as well as seek feedback, especially from those who will be using the tool most often.

The Key Difference was that the original tool only rolled up the “relative value units (RVU) patient acuity rating system. That original system didn’t translate the RVU to actual staff requirements.

The most important challenge here is ensuring that the tool is implemented quickly and effectively - in my opinion, good communication is what makes the difference. Be sure to measure and subsequently monitor impact, especially on quality of patient care.

A key part of using a tool is to review the data collected at set timepoints. This can reveal important findings, such as the need for more staff, changing appointment times to offpeak times, increasing interpretive services and more.
